摘要
目的探讨苯丙酮尿症(PKU)患儿发生智力低下的影响因素。方法选取2016年4月至2021年3月开封市儿童医院收治的280例PKU患儿,设计患儿基线资料调查表并记录研究所需资料,统计PKU患儿智力低下发生情况,根据神经心理发育量表(DST)评分,将智力低下的患儿纳入发生组,将智力正常的患儿纳入未发生组。将可能的影响因素纳入多因素logistic回归分析,找出PKU患儿发生智力低下的独立危险因素。结果280例PKU患儿中92例发生智力低下,发生率为32.86%(92/280)。发生组监护人受教育程度、家庭人均年收入、监护人早期遵医行为、起始治疗月龄与未发生组比较,差异有统计学意义(P<0.05);两组间其他因素相比,差异无统计学意义(P>0.05)。将性别、年龄、居住地、监护人性别、分娩方式、监护人受教育程度、家庭人均年收入、监护人早期遵医行为、起始治疗月龄作为自变量,将智力发育情况作为因变量,建立多因素logistic回归模型,结果显示,监护人受教育程度为初中及以下、家庭人均年收入<8000元、监护人早期遵医行为差、起始治疗月龄大是PKU患儿发生智力低下的独立危险因素(OR>1,P<0.05)。结论PKU患儿智力低下的发生与监护人受教育程度较低、家庭人均收入较低、监护人早期遵医行为差、起始治疗较晚等因素有关,临床可据此制定针对性的干预方案,以降低PKU患儿智力低下发生风险。
Objective To explore the influencing factors of mental retardation in children with phenylketonuria(PKU).Methods A total of 280 children with PKU received by Kaifeng Children’s Hospital from April 2016 to March 2021 were selected.The filling-in form of baseline data of children was designed,the data required for the study was recorded,and the occurrence of mental retardation in children with PKU was counted.According to the score of neuropsychological developmental screen test(DST),children with mental retardation were included in the occurrence group and children with normal intelligence were included in the non-occurrence group.Included possible influencing factors,through univariate and multivariate analysis,the influencing factors that might lead to mental retardation in children with PKU were found out.Results Among the 280 children with PKU,92 cases had mental retardation,and the incidence was 32.86%(92/280).There were differences in education level of guardians,family per capita annual income,early compliance behavior of guardians and initial treatment month age between the occurrence group and the non-occurrence group(P<0.05).There were no differences in other factors between the two groups(P>0.05).Taking gender,age,place of residence,gender of guardian,mode of delivery,education level of guardians,family per capita annual income,early compliance behavior of guardians and initial treatment month age as independent variables and intellectual development as dependent variables,a multifactor logistic regression model was established.The results showed that,junior high school and below education level of guardians,family per capita annual income<8000 yuan,poor early compliance behavior of guardians and older initial treatment months were the influencing factors of mental retardation in PKU children(OR>1,P<0.05).Conclusion The occurrence of mental retardation in children with PKU are related to low education level of guardians,low family per capita annual income,poor early compliance behavior of guardians,and later initial treatment months,according to this,targeted programs can be put forward to reduce the incidence of mental retardation in children with PKU.
