摘要
目的探讨动态对比增强磁共振成像(DCE-MRI)联合扩散加权成像(DWI)在乳腺病变诊断中的应用效果。方法选取2019年2月至2020年2月本院收治的乳腺病变患者60例,所有患者均行DCE-MRI检查及DWI检查,以病理结果为金标准,分析二者联合应用于乳腺病变中的诊断价值。结果病理结果显示,60例乳腺病变患者中17例为良性病变,43例为恶性病变;DCE-MRI联合DWI检出恶性41例,检出率为95.35%(41/43);联合检查检测出良性14例,检出率为82.35%(14/17)。经Kappa一致性检验,DCE-MRI及DWI联合检查乳腺病变结果与病理结果的一致性较好(Kappa=0.791,P=0.000);恶性病变患者流出型曲线分布率高于良性病变患者,差异有统计学意义(P<0.05);以1.01×10-3mm2/s作为良恶性病变诊断阈值,对照手术病理,乳腺良恶性病变DWI影像ADC值比较差异有统计学意义(P<0.05)。结论DCE-MRI联合DWI检查诊断乳腺病变具有较好的诊断效果,可为乳腺病变的临床诊治提供可靠的影像学依据。
Objective To explore the application effect of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)combined with diffusion-weighted imaging(DWI)in the diagnosis of breast lesions.Methods 60 patients with breast lesions in our hospital were selected From February 2019 to February 2020.All patients underwent DCE-MRI and DWI,and the diagnostic value of the combination of DCE-MRI and DWI was analyzed with pathological results as the gold standard.Results Pathological results showed that,among 60 cases of breast lesions,17 cases were benign and 43 cases were malignant.DCE-MRI combined with DWI detected 41 cases of malignancy,and the detection rate was 95.35%(41/43).14 cases were benign by combined examination,and the detection rate was 82.35%(14/17).Kappa consistency test showed that DCE-MRI com-bined with DWI showed a good consistency with pathological results(Kappa=0.791,P=0.000).The distribution rate of outflow curve in patients with malignant lesions was higher than that in patients with benign lesions,and the difference was statistically significant(P<0.05).Taking 1.01×10-3mm2/s as the diagnostic threshold for benign and malignant breast lesions,the ADC values of DWI images of benign and malignant breast lesions were signif-icantly different from those of surgical pathology(P<0.05).Conclusion DCE-MRI combined with DWI has good diagnostic effect in breast lesions,and can provide reliable imaging basis for clinical diagnosis and treatment of breast lesions.
