摘要
大量新型冠状病毒肺炎(COVID-19)的相关流行病学、西医机制研究及临床证据支持肺纤维化极有可能成为COVID-19严重并发症之一。随着国内疫情的有效控制,如何有效预防COVID-19患者恢复期肺纤维化的发生与发展成为目前亟待解决的问题。研究基于络病理论,结合COVID-19疾病特点及西医机制研究进展,提出COVID-19恢复期肺纤维化核心病因病机为"肺络亏虚,气血(络)失和,痰瘀阻络";"治疗前移,补虚通络"为核心治则,当贯穿疾病辨治始终;治疗应以"平"为目标,通过"调"之方式积极干预,努力使机体从"制"之状态恢复到"承"之稳态。此外,未来需要进一步深化理论探讨,开展相关科学研究与循证评价,为今后基于络病理论中医药治疗COVID-19恢复期肺纤维化提供可靠的循证证据,以更好的指导临床实践。
A large number of corona virus disease 2019(COVID-19)related epidemiology,Western medicine mechanism research and clinical evidence support that pulmonary fibrosis is very likely to become one of the serious complications of COVID-19. With the effective control of the epidemic situation in China,how to effectively prevent the occurrence and development of pulmonary fibrosis in convalescent patients with COVID-19 has become an urgent problem. Based on collateral disease theory,combined with the characteristics of COVID-19 and the research progress of Western medicine mechanism,the core etiology and pathogenesis of pulmonary fibrosis in the recovery stage of COVID-19 is“deficiency of lung collaterals,disharmony of Qi and blood,obstruction of collaterals by phlegm and blood stasis”. The goal is to actively intervene by means of“regulation”to make the body recover from the state of“control”to the state of“commitment”. In addition,in the future,it is necessary to further deepen the theoretical discussion,carry out relevant scientific research and evidence-based evaluation,so as to provide reliable evidence-based evidence for the treatment of convalescent pulmonary fibrosis of COVID-19 with traditional Chinese medicine based on collateral disease theory,so as to better guide clinical practice.
