摘要
目的:总结氯米芬(Clomiphene,CC)促排周期中联合补肾活血方对肾虚血瘀证卵泡发育不良综合征患者的卵泡发育的影响。方法:收集2018年1月—2021年1月就诊于深圳市中医院妇科的肾虚血瘀证卵泡发育不良综合征患者的临床资料,治疗上分别采用CC和CC+补肾活血方的促排方案,根据促排方案不同分为CC组和CC+中药组,针对344个促排周期进行回顾性研究。结果:CC+中药组在卵泡发育至扳机日的时间、优势卵泡平均直径、扳机日子宫内膜厚度方面、黄体期内膜血流均优于CC组,差异有统计学意义(P<0.05);CC+中药组卵泡成功排卵率高、小卵泡排卵周期少、未破裂卵泡黄素化综合征(LUFS)周期数少、无优势卵泡发育周期少,同时妊娠率明显高于CC组(P<0.05),两组在周期取消率比较上无统计学差异(P>0.05)。结论:补肾活血方介入CC促排可使肾虚血瘀证卵泡发育不良综合征患者的卵泡与内膜发育同步、卵泡合格率及排卵率高,值得临床推广。
Objective:To summarize the effect of Clomiphene(CC)combined with Bushen Huoxue Recipe on follicular development in patients with follicular dysplasia syndrome of kidney deficiency and blood stasis syndrome.Methods:The clinical data of patients with follicular dysplasia syndrome of kidney deficiency and blood stasis syndrome who were treated in the Department of Gynecology of Shenzhen Traditional Chinese Medicine Hospital from January 2018 to January 2021 were collected.Different ovulation promotion programs were divided into CC group and CC+traditional Chinese medicine group,and a retrospective study was carried out for 344 ovulation promotion cycles.Results:CC+Chinese medicine group was better than CC group in terms of time from follicle development to trigger day,average diameter of dominant follicle,endometrial thickness on trigger day,and luteal endometrial blood flow,and the differences were statistically significant(P<0.05);The CC+Chinese medicine group had higher successful ovulation rate,fewer small follicle ovulation cycles,fewer unruptured follicle luteinization syndrome(LUFS)cycles,and fewer developmental cycles of non-dominant follicles,and the pregnancy rate was significantly higher than that in the CC group(P<0.05).There was no statistical difference in the cycle cancellation rate between the groups(P>0.05).CONCLUSION:Bushen Huoxue Recipe intervening in CC ovulation induction can make the follicle and endometrial development synchronous in patients with follicular dysplasia syndrome of kidney deficiency and blood stasis syndrome,and the follicle qualified rate and ovulation rate are high,which is worthy of clinical promotion.
