摘要
目的 探究及观察括约肌间瘘管结扎术在单纯肛瘘患者中的疗效及对肛门功能的影响。方法 选取2019年1月—2021年1月的80例单纯肛瘘患者为研究对象,将其按照治疗方法分为对照组(切开挂线组)和观察组(括约肌间瘘管结扎术组),每组各40例。比较两组的临床疗效(总有效率)、手术前后的VAS评分、Wexner评分结果及肛肠动力学指标(直肠静息压、肛管静息压及肛管最大收缩压)。结果 术后1个月及3个月观察组的总有效率分别为97.50%及100%,显著高于对照组的80.00%及87.50%,差异有统计学意义(P <0.05),术前两组的VAS评分、Wexner评分结果及肛肠动力学指标比较,差异无统计学意义(P> 0.05),术后7 d及14 d观察组的VAS评分均显著低于对照组,术后1个月及3个月观察组的Wexner评分结果及肛肠动力学指标显著优于对照组,差异有统计学意义(P <0.05)。结论 括约肌间瘘管结扎术在单纯肛瘘患者中的疗效较好,且对肛门功能的损伤控制较好,因此在单纯肛瘘患者中的应用价值较高。
Objective To investigate and observe the effect of ligation of intersphincteric fistula tract in patients with simple anal fistula and influence for anal function. Methods A total of 80 patients with simple anal fistula from January 2019 to January 2021 were chosen as the research object, and they were divided into control group(incisionthread-drawing procedure group) and observation group(ligation of intersphincteric fistula tract group) with 40 cases in each group by the treatment methods. Then the clinical effect(total effective rates),VAS score, Wexner score and anorectal dynamics indexes(rectal resting pressure, anal resting pressure and maximum anal systolic pressure) before and after the operation of two groups were compared.Results The total effective rates of observation group at first and third month after the operation were respectively 97.50% and 100%,and they were significantly higher than 80.00% and 87.50% of control group, the differences of those items of two groups were statistically significant(P < 0.05), the VAS score, Wexner score and anorectal dynamics indexes of two groups before the operation were compared, the differences of those items of two groups were not statistically significant(P > 0.05), the VAS score of observation group at seventh and fourteenth day after the operation were significant lower than those of control group, the Wexner score and anorectal dynamics indexes of observation group at first and third month after the operation were significantly better than those of control group, the differences of those items of two groups were statistically significant(P < 0.05). Conclusion The effect of ligation of intersphincteric fistula tract in patients with simple anal fistula is better, and its control role for the injury of anal function are better,so its application value in the patients with simple anal fistula is higher.
