摘要
目的:观察早期肠内营养耐受性分级护理在重症监护室(ICU)脓毒血症患者中的应用效果。方法:选取60例脓毒血症患者为研究对象,按照随机数字表法分为研究组与对照组各30例。对照组给予常规肠内营养护理,研究组在对照组基础上给予早期肠内营养耐受性分级护理,比较两组热卡达标率、喂养不耐受率、血清白蛋白(ALB)水平、血清前白蛋白(PA)水平和急性生理学与慢性健康状况评分系统Ⅱ(APACHEⅡ)评分。结果:护理第4、7天,研究组热卡达标率高于对照组,喂养不耐受率低于对照组,差异有统计学意义(P<0.05);护理第4、7天,研究组ALB、PA水平均高于对照组,差异有统计学意义(P<0.05);护理第4、7天,研究组APACHEⅡ评分均低于对照组,差异有统计学意义(P<0.05)。结论:在常规肠内营养护理基础上采用早期肠内营养耐受性分级护理可提高ICU脓毒血症患者热卡达标率和ALB、PA水平,以及降低喂养不耐受率和APACHEⅡ评分,效果优于单纯常规肠内营养护理。
Objective:To observe application effects of early enteral nutrition tolerance grading nursing in ICU patients with sepsis.Methods:60 patients with sepsis were selected as the research objects and were divided into study group and control group according to the random number table method,30 cases in each.The control group was given routine enteral nutrition nursing,while the study group was given early enteral nutrition tolerance grading nursing on the basis of that of the control group.The calorie compliance rate,the feeding intolerance rate,the serum albumin(ALB)level,the serum prealbumin(PA)level and the acute physiology and chronic health evaluation scoring system Ⅱ(APACHEⅡ)score were compared between the two groups.Results:On the 4th and 7th day of nursing,the calorie compliance rate of the study group was higher than that of the control group;the feeding intolerance rate was lower than that of the control group;and the differences were statistically significant(P<0.05).On the 4th and 7th day of the nursing,the levels of ALB and PA in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,on the 4th and 7th day of nursing,the APACHEⅡ score of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusions:On the basis of the routine enteral nutrition nursing,the early enteral nutrition tolerance grading nursing can improve the calorie compliance rate and the ALB and PA levels,and reduce the feeding intolerance rate and the APACHEⅡ score in the patients with sepsis in ICU.Moreover,it is superior to single routine enteral nutrition nursing.
