老年糖尿病视网膜病变患者血脂水平变化及与疾病严重程度关系研究 被引量:3

Changes of serum lipids in elderly diabetic retinopathy patients and their relationship with severity of disease

摘要 目的探讨老年糖尿病视网膜病变(DR)患者血脂水平变化及其与疾病严重程度的关系。方法回顾性分析郑州市骨科医院2019年1月至2020年12月收治的老年DR患者150例作为研究对象,将其作为DR组,其中非增殖性糖尿病视网膜病变(NPDR)65例(A组),增殖性糖尿病视网膜病变85例(B组),并纳入同期体检健康者60例作为对照组。检测各组受试者总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)水平差异,并以Spearman分析各检测指标与疾病严重程度的关系,采用Logistic回归分析DR可能的危险因素。结果DR组患者TG、TC、LDL-C水平分别为(2.13±0.12)mmol/L、(4.48±0.61)mmol/L、(3.31±0.23)mmol/L,高于对照组,HDL-C水平为(1.11±0.21)mmol/L,低于对照组,差异有统计学意义(P<0.05);B组患者TG、TC、LDL-C水平分别为(3.71±0.36)mmol/L、(5.98±0.12)mmol/L、(4.38±0.13)mmol/L,高于A组,HDL-C水平为(0.80±0.09)mmol/L,低于A组,差异有统计学意义(P<0.05);Spearman相关性分析表明,糖尿病视网膜病变程度与TC、TG、LDL-C水平分别呈正相关,差异有统计学意义(r=0.512,0.609,0571;P<0.05),而与HDL-C水平呈显著负相关,差异有统计学意义(r=-0.419;P<0.05);Logistic回归分析显示,TC、TG、HDL-C、LDC-C均是诱导DR发生的潜在危险因素,差异有统计学意义(P<0.05)。结论血脂代谢异常可诱导DR的发生及发展,动态监测血脂变化对临床提高DR防治有积极作用。 Objective To investigate the changes of serum lipids in elderly patients with Diabetic retinopathy(DR)and their relationship with the severity of the disease.Methods 150 cases of elderly DR patients admitted in January 2019~2020 in December as the research object,as the DR group,including 65 cases of Non proliferative diabetic retinopathy(NPDR),A group,85 cases of proliferative diabetic retinopathy(B group),and 60 cases of healthy persons in the same period as the control group.The levels of total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDL-C)and low density lipoprotein(LDL-C)were detected.The relationship between each detection index and disease severity was analyzed by Spearman,and the possible risk factors of Dr were analyzed by logistic regression.Results The levels of TG,TC and LDL-C in the DR group were(2.13±0.12)mmol/L,(4.48±0.61)mmol/L and(3.31±0.23)mmol/L,respectively,which were higher than those in the control group,and the HDL-C level was(1.11±0.21)mmol/L,lower than the control group,the difference was statistically significant(P<0.05);the levels of TG,TC and LDL-C in group B were(3.71±0.36)mmol/L,(5.98±0.05)0.12)mmol/L,(4.38±0.13)mmol/L,higher than group A,HDL-C level was(0.80±0.09)mmol/L,lower than group A,the difference was statistically significant(P<0.05);Spearman correlation analysis showed that the degree of diabetic retinopathy was positively correlated with the levels of TC,TG,and LDL-C(r=0.512,0.609,0571;P<0.05),but with HDL-C level was significantly negatively correlated(r=-0419;P<0.05);Logistic regression analysis showed that:TC,TG,HDL-C,LDC-C are all potential risk factors for inducing DR(P<0.05).Conclusion Abnormal blood lipid metabolism can induce the occurrence and development of DR,and its level changes are closely related to the degree of inflammation.Dynamic monitoring of blood lipid changes has a positive effect on the clinical improvement of DR.
