摘要
目的分析腹膜透析患者营养状况的影响因素。方法103例行腹膜透析治疗的患者为研究对象,分析患者的营养状况及影响因素。结果103例患者中,41例营养正常,33例轻中度营养不良,29例重度营养不良,营养不良发生率为60.19%。营养正常患者和营养不良患者的年龄、透析病程、医疗付费方式、家庭年收入、焦虑、抑郁比较,差异具有统计学意义(P<0.05);营养正常患者和营养不良患者的性别、疾病类型比较,差异无统计学意义(P>0.05)。经Logistic回归分析显示,年龄≥60岁、透析病程≥60个月、自费医疗、家庭年收入<3万元、有焦虑、有抑郁均为腹膜透析患者发生营养不良的独立危险因素(OR=2.121、3.873、3.152、2.591、1.664、1.885,P<0.05)。结论年龄≥60岁、透析病程≥60个月、自费医疗、家庭年收入<3万元、有焦虑、有抑郁均为腹膜透析患者发生营养不良的独立危险因素,临床治疗过程中应重视影响患者营养状况的相关危险因素。
Objective To analyze the influencing factors of nutritional status of patients undergoing peritoneal dialysis.Methods 103 patients undergoing peritoneal dialysis were taken as the research subjects.The nutritional status and the influencing factors of patients were analyzed.Results Among the 103 patients,there were 41 cases with normal nutrition status,33 cases with mild to moderate malnutrition,and 29 cases with severe malnutrition.The total incidence of malnutrition was 60.19%.The differences in age,dialysis course,medical payment mode,annual family income,anxiety and depression between patients with normal nutrition and patients with malnutrition were statistically significant(P<0.05).There was no statistically significant difference in gender and disease type between patients with normal nutrition and patients with malnutrition(P>0.05).Logistic regression analysis showed that age≥60 years,dialysis course≥60 months,self-financed medical care,annual family income<30000 yuan,anxiety,and depression were all independent risk factors for malnutrition in peritoneal dialysis patients(OR=2.121,3.873,3.152,2.591,1.664,1.885;P<0.05).Conclusion Age≥60 years,dialysis course≥60 months,self-financed medical care,annual family income<30000 yuan,anxiety,and depression are all independent risk factors for malnutrition in peritoneal dialysis patients.In clinical treatment,related risk factors that affect nutritional status of patients should be payed attention to.
