脉搏血氧饱和度监测用于临床筛查新生儿先天性心脏病的价值 被引量:1

摘要 目的研究脉搏血氧饱和度监测用于临床筛查新生儿先天性心脏病的价值。方法回顾性选择东莞广济医院儿科2020年1月至2021年8月接收的600例新生儿作为研究对象,采用脉搏血氧饱和仪监测,以心脏彩色超声心动图检查结果作为诊断金标准,并将新生儿分为正常健康组(500例)、单纯先天性心脏病组(70例)、复杂先天性心脏病组(30例)。比较三组的胎龄、体重、脉搏血氧饱和度,并分析脉搏血氧饱和度监测对新生儿先天性心脏病诊断效能。结果单纯先天性心脏病组新生儿的胎龄短于正常健康组、复杂先天性心脏病组,体重低于正常健康组、复杂先天性心脏病组,差异有统计学意义(P<0.05);三组脉搏血氧饱和度两两比较,差异有统计学意义(P<0.05)。脉搏血氧饱和度对新生儿先天性心脏病诊断的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为95.00%(95/100)、96.00%(480/500)、95.83%(575/600)、82.61%(95/115)、98.97%(480/485)。结论脉搏血氧饱和度监测对早期筛选先天性心脏病患儿具有较高的应用价值。 Objective To study the value of pulse oxygen saturation monitoring in clinical screening of neonatal congenital heart disease.Methods A total of 600 newborns received by Department of Pediatrics,Dongguan Guangji Hospital from January 2020 to August 2021 were retrospectively selected as the research objects.They were monitored by pulse oxygen saturation meter,and the results of cardiac color echocardiography were used as the diagnostic gold standard.The newborns were divided into the normal health group(500 cases),the simple congenital heart disease group(70 cases)and the complex congenital heart disease group(30 cases).The gestational age,weight,pulse oxygen saturation of three groups were compared,and the diagnostic efficacy of pulse oxygen saturation monitoring for neonatal congenital heart disease was analyzed.Results The gestational age of newborns in simple congenital heart disease group was shorter than that in normal healthy group and complex congenital heart disease group,and weight was lower than that in normal healthy group and complex congenital heart disease group,the differences were statistically significant(P<0.05).The pulse oxygen saturation of the three groups was compared in pairs,and the difference was statistically significant(P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of pulse oxygen saturation in the diagnosis of neonatal congenital heart disease were 95.00%(95/100),96.00%(480/500),95.83%(575/600),82.61%(95/115)and 98.97%(480/485),respectively.Conclusion Pulse oxygen saturation monitoring has high application value in earl y screening of children with congenital heart disease.
