摘要
食管癌是最常见的上消化道肿瘤之一,患者的营养状况较其他类型肿瘤更差。流行病学研究表明,不良饮食习惯和微量元素缺乏是食管癌发生的危险因素。血液中的一些指标可以间接反映机体的营养状况。营养和免疫状态的好坏可显著影响肿瘤细胞在体内的生长与增殖。肿瘤细胞对机体的免疫作用有双重性,既可以表达诱导免疫反应的分子,进而影响其侵袭和转移,也可以表达抑制免疫细胞活化和增殖的因子,从而避开免疫反应对肿瘤的制约。免疫逃逸在食管癌的侵袭中有重要作用,阐明免疫逃逸的机制对免疫靶向治疗有重要指导意义。本文总结了营养、免疫状况与食管癌患者生存及预后相关的研究进展。
Esophageal cancer is one of the most common upper gastrointestinal tumors and is less nutritionally present than other types of tumors.A large number of epidemiological studies have shown that poor eating habits and deficiencies of various trace elements are risk factors for the development of esophageal cancer.Some indicators in the blood can indirectly reflect the nutritional status of the body.The quality of nutrition and immune status can significantly affect the growth and proliferation of tumor cells in the body.It has been found that the immune effect of tumor cells on the body is dual:it can express molecules that induce the immune response,which in turn interferes with its invasion and metastasis;it can also express factors that inhibit the activation and proliferation process of immune cells,thereby avoiding the immune response interfering with the progression of the tumor,and the immune escape mechanism plays an important role in the aggressive behavior of esophageal cancer,which plays an important role in guiding the clinical immune targeted therapy.In this paper,we summarize the research progress of nutrition and immune status on the survival and prognosis of patients with esophageal cancer.
