摘要
目的:观察运脾化痰通窍方联合迎香穴按摩治疗儿童腺样体肥大脾虚痰阻证的临床疗效。方法:选取2017年1月至2019年8月收治的170例符合脾虚痰阻证腺样体肥大诊断标准的患儿作为研究对象,随机分为治疗组和对照组。治疗组采用中药联合外治疗法,以运脾化痰通窍方配合迎香穴按摩,对照组采用孟鲁司特钠咀嚼片配合中药安慰剂,2组均连续治疗3个月,第4个月进行随访,最长随访期6个月,观察疾病主要症状复发率及伴随疾病发生率。结果:治疗组总有效率为97.65%,高于对照组总有效率81.18%,差异有统计学意义(P<0.01)。2组治疗后主要症状积分、A/N值积分均明显低于治疗前,且治疗组积分明显低于对照组(P<0.05,P<0.01)。随访6个月发现,治疗组患儿主要症状复发率(如鼻塞、打鼾、张口呼吸)及过敏性鼻炎发生率均显著低于对照组(P<0.05,P<0.01)。结论:运脾化痰通窍方联合中医外治法治疗儿童腺样体肥大脾虚痰阻证,可明显改善患儿临床症状,缩小腺样体体积,降低主要症状复发率。
Objective:To observe the clinical efficacy of Yunpi Huatan Tongqiao Formula combined with Yingxiang acupoint massage in the treatment of Spleen Deficiency and Phlegm Obstruction Syndrome of adenoid hypertrophy in children.Methods:A total of 170 children who met the diagnostic criteria of Spleen Deficiency and Phlegm Obstruction Syndrome of adenoid hypertrophy from January 2017 to August 2019 were randomly divided into treatment group and control group.The treatment group was treated with Yunpi Huatan Tongqiao recipe combined with Yingxiang acupoint massage.The control group was treated with montelukast sodium chewable tablets combined with traditional Chinese medicine placebo.Both groups were treated continuously for 3 months and followed up for 4 months.The longest follow-up period was 6 months.The recurrence rate of main symptoms and the incidence of concomitant diseases were observed.Results:the total effective rate of the treatment group was 97.65%,which was higher than that of the control group(81.18%),and the difference was statistically significant(P<0.01).After treatment,the scores of main symptoms and A/N value in the two groups were lower than those before treatment,and the scores in the treatment group were lower than those in the control group,and the difference was statistically significant(P<0.05,P<0.01).After follow-up for 6 months,the recurrence rate and the incidence of concomitant symptoms and allergic rhinitis in the treatment group were significantly lower than those in the control group(P<0.05,P<0.01).Conclusion:The treatment of adenoid hypertrophy,spleen deficiency and phlegm obstruction syndrome in children with Yunpi Huatan Tongqiao recipe combined with external treatment of traditional Chinese medicine can significantly improve the clinical symptoms,reduce the volume of adenoid and reduce the recurrence rate of symptoms.
