摘要
目的:探讨用新型体外定位器配合空心拉力螺钉治疗PauwelsⅠ型和Ⅱ型股骨颈骨折的效果。方法:将近年来西南医科大学附属中医医院收治的42例PauwelsⅠ型和Ⅱ型股骨颈骨折患者纳入本研究。随机将其分为常模组和讨论组,每组各有患者21例。对常模组患者进行常规的空心拉力螺钉内固定术,讨论组患者在常模组的基础上应用新型体外定位器进行手术。比较两组患者的各项临床指标。结果:与常模组患者相比,讨论组患者手术的时间更短,术中的出血量更少,术后1周的VAS评分更低,差异有统计学意义(P<0.05)。与常模组相比,讨论组患者术后1个月、3个月和6个月的Harris髋关节功能评分均更高,差异有统计学意义(P<0.05)。讨论组患者对治疗效果的总满意率为95.24%,常模组患者对治疗效果的总满意率为80.59%,二者相比差异无统计学意义(P>0.05)。术后两组患者的手术切口均达到甲级愈合,均未出现并发症。术后6个月对两组患者进行X线复查显示其骨折断端对位对线均良好。结论:用新型体外定位器配合空心拉力螺钉治疗PauwelsⅠ型和Ⅱ型股骨颈骨折的效果显著,能有效缩短手术的时间,减少患者术中的出血量,减轻其术后的疼痛感,促进其骨折端的愈合和髋关节功能的恢复。
Objective:To investigate the effect of a new external positioner combined with a hollow lag screw in the treatment of femoral neck fractures of Pauwels TYPEⅰand typeⅱ.Methods:Forty-two patients with femoral neck fracture of Pauwels TYPEⅰand typeⅱadmitted to the Affiliated Hospital of Traditional Chinese Medicine of Southwest Medical University in recent years were included in this study.They were randomly divided into norm group and discussion group,with 21 patients in each group.Patients in the normal group were treated with conventional hollow lag screw internal fixation,and patients in the discussion group were treated with a new external positioner on the basis of the normal group.The clinical indicators of the two groups were compared.Results:Compared with the norm group,patients in the discussion group had shorter operation time,less intraoperative blood loss,and lower VAS score 1 week after surgery,with statistically significant differences(P<0.05).Compared with the norm group,Harris hip function scores were higher at 1,3 and 6 months after surgery in the discussion group,and the difference was statistically significant(P<0.05).The total satisfaction rate of the treatment effect in the discussion group was 95.24%,and the total satisfaction rate of the treatment effect in the norm group was 80.59%,and there was no statistical difference between the two groups(P>0.05).The incisions in both groups were grade A healing and no complications occurred.Six months after the operation,X-ray reexamination of the two groups showed that the alignment of the fracture end was good.Conclusion:The treatment of Pauwels TYPEⅰand typeⅱfemoral neck fractures with a new external positioner combined with a hollow lag screw has a significant effect,which can effectively shorten the operation time,reduce the amount of intraoperative blood loss,relieve the postoperative pain,and promote the hea ling of the fracture end and the recovery of hip function.
