摘要
正所谓“诸病易治,咳嗽难医”。顽固性咳嗽迁延难愈,病程可达数月甚至经年。顽固性咳嗽虽然没有明显的器质性病变,但是严重影响患者的生活质量,也增加了罹患其他疾病的风险。目前西医治疗以对症治疗为主,治疗方式和治疗效果有限。中医治疗咳嗽疗效确切,提出了很多治疗思路、方法和方剂,总结了很多治疗经验,为本病的治疗做出了巨大贡献。临床中相当一部分咳嗽是由寒湿为患、气机逆乱所引起的,针对此类顽固性咳嗽,可采用温化寒湿、调畅气机的治则治法,方选小青龙汤温阳化湿,温肺化饮;选小柴胡汤和解少阳枢机,调畅内外上下气机的运动。此类型咳嗽常有如下特点:症状方面,咳嗽呈阵发性、刺激性;咽痒即咳、躺下即咳、讲话即咳、进食即咳;痰稀白或干咳无痰,喉中哮鸣有声或气喘;咳甚呕吐、咳则胁痛。舌脉方面,舌淡嫩或胖,苔白腻或水滑,脉沉弦或弦滑。六经辨证方面可辨为太阳少阳合病夹饮。临证使用时抓住寒湿为患、气机逆乱的病机据证加减即可取得良效。
It is called“all diseases are easy to treat,cough is difficult to cure”.Intractable cough is difficult to be prolonged,and the course of disease can reach months or even years.Although intractable cough has no obvious organic lesions,it seriously affects the quality of life of patients,and increases the risk of other diseases.At present,Western medicine treatment is mainly symptomatic treatment,and the treatment method and treatment effect are limited.The curative effect of TCM for cough is accurate,and many treatment ideas,methods and prescriptions are put forward.Many experience of treatment are summarized,which has made great contribution to the treatment of the disease.A large part of cough in clinical is caused by cold and wet,and the Qi(气)is in disorder.For such intractable cough,the treatment method of warming cold and dampness and regulating the Qi can be adopted.The way is to select the Xiaoqinglong decoction(小青龙汤)to warm Yang(阳)and warm lung;to select the Xiaochaihu decoction(小柴胡汤)compromise Shaoyang(少阳)axial machine to regulate the movement of the upper and lower Qi inside and outside.Cough of this type often has the following characteristics:symptoms:cough is paroxysmal and irritating;itchy throat is cough,lying down cough,speech cough,eating cough;phlegm is thin white or dry cough has no phlegm,and the throat wheezes with sound or asthma;cough vomit,cough is threatening pain.Tongue vein aspect:tongue is tender or fat,moss is white and greasy or slippery,pulse is heavy or string slippery.The six classics can be identified as the Taiyang(太阳)Shaoyang disease mixed retained fluid.When using the temporary certificate,it can get good effect by grasping the pathogenesis of cold and wet,and the Qi is in disorder.
