重庆市荣昌区不同职业人群布鲁氏菌病影响因素分析 被引量:2

Analysis of influencing factors of brucellosis in different occupational groups in Rongchang district of Chongqing

摘要 目的了解重庆市荣昌区布鲁氏菌病血清抗体阳性检出情况及其影响因素,为政府制定布鲁氏菌病防控措施提供科学依据。方法对2016—2020年布鲁氏菌病监测点不同职业人群抽血采样,采用虎红平板凝集试验初筛,阳性者用试管凝集试验确诊。对布鲁杆菌抗体初筛阳性相关影响因素进行单因素和多因素Logistic回归分析。结果2016—2020年共监测不同职业人群1081人,血清学抗体阳性37例、平均抗体阳性率3.42%;单因素分析结果显示,厨师血清抗体阳性率最高(8.20%),血清抗体阳性率随年龄的增长(趋势χ^(2)=4.584)和从业时间的增加(趋势χ^(2)=18.472)均呈下降趋势(P<0.05);多因素Logistic回归分析显示,引起不同职业人群血清抗体阳性的主要传染源是染疫羊(OR=3.546,95%CI:1.345~9.350),血清抗体阳性率养殖人员(OR=12.604,95%CI:2.962~53.645)、皮毛乳肉加工人员(OR=16.727,95%CI:2.235~125.129)和厨师(OR=22.100,95%CI:4.147~117.781)高于兽医。结论荣昌区不同职业人群血清布鲁杆菌抗体阳性主要是通过接触染疫羊而引起;应加大厨师、养殖人员和皮毛乳肉加工人员的健康宣传教育力度,提高医疗机构对布鲁氏菌病的诊断和报告能力,落实好动物检疫制度。 Objective To understand the positive detection of brucellosis antibody and the related influencing factors in Rongchang district,Chongqing,and provide scientific basis for the government to formulate prevention and control measures of brucellosis.Methods The blood samples were collected from different occupational populations in the surveillance sites of brucellosis from 2016 to 2020.The tiger red plate agglutination test(RBPT)was used for preliminary screening,and the positive cases were confirmed by standard agglutination test(SAT).The related influencing factors of the positive cases in the brucellosis antibody screening were analyzed by univariate and multivariate logistic regression.Results A total of 1081cases were monitored from 2016 to 2020 with the average positive rate of 3.42%.The results of univariate analysis showed that the positive rate of antibody in serum was highest in chefs(8.20%),and presented downtrend(P<0.05)with the age(trendχ^(2)=4.584)and the working time(trendχ^(2)=18.472).The multivariate logistic regression showed that the main infection source of causing serum antibody positive in different occupational groups was the infected sheep(OR=3.546,95%CI:1.345-9.350).The positive rates of antibody in serum were higher in the breeders(OR=12.604,95%CI:2.962~53.645),processing persons of fur,milk and meat(OR=16.723,95%CI:2.235-125.129),and chefs(OR=22.100,95%CI:4.147-117.781)were higher than in the veterinarians.Conclusions The positive of brucella antibody in the serum of different occupational groups is mainly caused by exposure to the infected sheep in Rongchang district.The health publicity and education should be strengthened in chefs,breeders and processors of fur,milk and meat.The ability of diagnosis and reporting to brucellosis in the medical institutions should be improved,and the animal inspection and quarantine system should be implemented.
