摘要
目的:研究穴位埋线结合长蛇灸治疗寒湿痹阻型强直性脊柱炎的临床疗效。方法:将选取的60例寒湿痹阻型强直性脊柱炎随机分为治疗组和对照组。治疗组给予穴位埋线(厥阴俞、心俞、肝俞、胆俞、三焦俞及肾俞6组共12个穴位)和长蛇灸,穴位埋线采用9号一次性埋线针,将1 cm胶原蛋白线放入针管,采用垂直进针,进针深度为1.5 cm,将蛋白线埋入筋膜层,长蛇灸是以背部大椎到肾俞段,使用艾绒、生姜等药物进行治疗,每周治疗1次,共治疗4周。对照组采用穴位埋线,治疗穴位和方法同治疗组,每周1次,共4次。观察两组治疗前后的症状量化评分、VAS疼痛评分、Schober试验、枕墙距、血沉(ESR)及C反应蛋白(CRP)的变化。结果:两组的症状量化评分、VAS疼痛评分、枕墙距、血沉(ESR)及C反应蛋白(CRP)均低于治疗前,差异具有统计学意义(P<0.05),Schober试验指标要高于治疗前,差异具有统计学意义(P<0.05)。在治疗后,治疗组的症状量化评分、VAS疼痛评分、枕墙距、血沉(ESR)及C反应蛋白(CRP)均低于对照组,差异具有统计学意义(P<0.05);治疗组Schober试验指标要高于对照组,差异具有统计学意义(P<0.05);治疗组的总有效率86.7%(26/30),高于对照组的76.7%(23/30,P<0.05)。结论:穴位埋线结合长蛇灸治疗强直性脊柱炎有着明显疗效,可明显减轻患者疼痛,改善生命体征,提高生活幸福感。
Objective:To study the clinical effect of acupoint catgut embedding combined with long snake moxibustion in the treatment of ankylosing spondylitis(AS)of cold-dampness obstruction.Methods:The selected 60 AS cases of cold-dampness obstruction were randomly divided into the treatment group and the control group.The treatment group was treated with acupoint catgut embedding(Jueyinshu,Xinshu,Ganshu,Danshu,Sanjiaoshu,Shenshu)and long snake moxibustion.The acupoint embedding uses a No.9 disposable embedding needle.Put the 1cm collagen thread into the needle tube.Use the vertical needle insertion with a depth of 1.5cm and embed the protein thread into the fascia layer.Long snake moxibustion is using moxa,ginger and other drugs on the back from Dazhui to Shenshu.The treatment is performed once a week for 4 weeks.The control group was only treated with acupoint catgut embedding,the points and the methods were the same as those in the treatment group.Symptom quantitative score,VAS pain score,Schober test indexes,occipital wall distance,erythrocyte sedimentation rate(ESR)and C-reactive protein(CRP)were observed before and after the treatment in the two groups.Results:Symptom quantitative score,VAS pain score,occipital wall distance,and the levels of ESR and CRP were lower(P<0.05),but the Schober test indexes were higher after the treatment than those before the treatment in the two groups(P<0.05).After the treatment,symptom quantitative score,VAS pain score,occipital wall distance,and the levels of ESR and CRP were lower(P<0.05),and the Schober test indexes were higher in the treatment group than those in the control group(P<0.05).The total effective rate of the treatment group was 86.7%(26/30),which was significantly higher than 76.7%(23/30)of the control group(P<0.05).Conclusion:Acupoint catgut embedding combined with long snake moxibustion has obvious curative effect in the treatment of AS,which can significantly reduce pain of the patients,improve life signs and life happiness.
