摘要
目的:观察泼尼松、头孢拉定、溴己新鼻内镜下咽鼓管插管输注治疗复发性分泌性中耳炎的效果。方法:80例复发性分泌性中耳炎患者,按就诊时间分为联合组及基础组,各40例。基础组给予头孢拉定+溴己新鼻内镜下咽鼓管插管输注治疗,联合组在基础组治疗基础上加用泼尼松治疗,观察两组患者疗效、炎性因子[白细胞介素-6(IL-6)、白细胞介素-8(IL-8)]水平及中耳功能。结果:治疗后,联合组疗效高于基础组(P<0.05);两组患者血清及中耳积液中内毒素水平均有所降低,且联合组低于基础组(P<0.05);联合组CD4^(+)、CD8^(+)及CD4^(+)/CD8^(+)水平均高于基础组(P<0.05);两组气导听阀、骨导听阀值均有所降低,且联合组高于基础组(P<0.05)。结论:泼尼松、头孢拉定、溴己新鼻内镜下咽鼓管插管输注治疗复发性分泌性中耳炎可以减轻炎症反应,促进患者听力功能的恢复。
Objective:To observe the effects of prednisone,cefradine and bromhexine infusion through intubation of eustachian tube under nasal endoscope on patients with recurrent secretory otitis media.Methods:80 patients with recurrent seretory otitis media were divided into the combined therapy group and the basic therapy group according to the visiting time,with 40 cases in each group.The basic therapy group was given cefradine plus bromhexine infusion through intubation of eustachian tube under nasal endoscope.The combined therapy group was treated with prednisone on the basis of the basic therapy group.The curative effect,levels of inflammatory factors[IL-6 and IL-8]and middle ear function were observed between the two groups.Results:After treatment,the curative effect of the combined therapy group was higher than that of the basic therapy group(P<0.05);the endotoxin levels in serum and middle ear effusion of both groups were decreased,and the combined therapy group was lower than that of the basic therapy group(P<0.05);the levels of CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)were higher than those in the basic therapy group(P<0.05);and the air conduction valve and bone conduction threshold were decreased in both groups,and the combined therapy group was higher than that in the basic group(P<0.05).Conclusion:Intranasal intubation infusion of prednisone,cefradine and bromhexine in the treatment of recurrent secretory otitis media can alleviate inflammation and promote the recovery of hearing function in patients.
