摘要
目的通过对某三级医院2018—2020年3732例急性心肌梗死(acute myocardial infarction,AMI)患者住院费用进行分析,找出影响住院费用的因素,为制定合理的支付标准提供依据。方法运用描述性统计分析、单因素分析以及多元回归,分析对住院费用影响有统计学意义的变量(P<0.05)。结果2018—2020年AMI患者住院费用呈上升趋势,2020年比2018年增长9.15%。平均住院费用43178.58元,诊断费用占比50.73%,为住院费用构成中最高;其次是药费用,占比23.97%;手术费用占比12.22%;排名第4的是治疗费用,占比9.81%;比例最低的是其他费用,占比7.14%。年龄、住院天数、出院科别、有无手术、手术方式及离院方式均对住院费用有影响,其中影响程度最大的是有无手术和住院天数。结论应建立健全单病种控制机制,不断完善胸痛中心的建设,以价值医疗为导向,合理检查、合理用药,缩短患者平均住院日,减轻患者经济负担。
Objective To find out the factors affecting the hospitalization costs,and to provide the basis for the development of reasonable payment standards,through the analysis of 3732 cases of acute myocardial infarction patients from 2018 to 2020 in a tertiary hospital of a certain city.Methods Descriptive statistical analysis,univariate analysis and multiple regression analysis were used to find out the variables with statistical significance(P<0.05).Results The hospitalization cost of AMI was on the rise from 2018 to 2020,and increased by 9.15%in 2020 compared with that in 2018.The average hospitalization cost was 43178.58 yuan,and the diagnosis cost accounted for 50.73%,which was the highest among the hospitalization cost components.Drug costs followed,accounting for 23.97%;operation cost accounted for 12.22%;the fourth was the cost of treatment,accounting for 9.81%;the lowest proportion was other expenses,accounting for 7.14%.Age,length of stay,department of discharge,operation,operation method and mode of discharge had influence on hospitalization expenses,among which operations and length of stay had the greatest influence.Conclusion It is necessary to establish and perfect the control mechanism of single disease and constantly improve the construction of chest pain center,to take value-oriented medical treatment as the guidance,with reasonable examination and rational drug use,shorten the average hospital stay of patients,and reduce the economic burden of patients.
