摘要
目的:探讨全髋关节置换术和半髋关节置换术应用在老年股骨颈骨折中的临床效果。方法:选择新邵县中医医院2019年6月-2020年6月收治的股骨颈骨折患者130例作为研究对象,按照手术方式将患者分为全髋组(n=110)与半髋组(n=20),全髋组行全髋关节置换术,半髋组实施半髋关节置换术,比较两组手术时间、手术出血量、住院时间,分析两组泌尿系统感染情况,评价分析聚乙烯内衬蠕变情况、Harris评分。结果:半髋组手术时间、住院时间短于全髋组,手术出血量少于全髋组,差异有统计学意义(P<0.05);全髋组聚乙烯内衬蠕变2例,半髋组聚乙烯内衬蠕变1例,差异无统计学意义(χ^(2)=0.760 0,P>0.05);全髋组泌尿系统感染率(2.73%)低于半髋组(10.00%),差异有统计学意义(P<0.05);两组手术后、术后1年Harris评分差异无统计学意义(P>0.05)。结论:HA与THA临床应用效果各有优势,全髋关节置换术手术时间长、出血稍多。半髋关节置换术手术时间短,出血量少,风险相对减小,临床应根据患者实际情况选择手术方法。
Objective:To investigate the clinical effects of total hip arthroplasty and hemiarthroplasty in elderly femoral neck fractures.Method:A total of 130 patients with femoral neck fractures admitted to Xinshao County Traditional Chinese Medicine Hospital from June 2019 to June 2020 were selected as the research objects.The patients were divided into total hip group (n=110) and half hip group (n=20).Total hip arthroplasty was performed in the total hip group,and hemiarthroplasty was performed in the half hip group.The operation time,surgical blood loss,and hospital stay were compared between the two groups.The urinary system infection of the two groups was analyzed,and the polyethylene lining creep,Harris score were evaluated and analyzed.Result:The operation time and hospital stay of the half hip group were shorter than those of the total hip group,and the blood loss during the operation was less than that of the total hip group,the differences were statistically significant (P<0.05).There were 2 cases of polyethylene liner creep in the total hip group,there was 1 case of polyethylene liner creep in the hip group,and the difference was not statistically significant (χ;=0.760 0,P>0.05).The urinary system infection rate in the total hip group (2.73%) was lower than 10.00% in the half hip group,and the difference was statistically significant (P<0.05).There were no significant differences in Harris scores between the two groups after surgery and 1 year after surgery (P>0.05).Conclusion:HA and THA have their respective advantages in clinical application.Total hip replacement has long operation time and slightly more bleeding.Half hip replacement has short operation time,less bleeding and relatively reduced risk.Surgical methods should be selected according to the actual situation of patients.
