摘要
目的:观察舒适化浅镇静护理在重症监护室(ICU)重症脑外伤机械通气患者中的应用效果。方法:选取86例ICU重症脑外伤机械通气患者为研究对象,按照随机数字表分为对照组和观察组各43例。对照组采用常规护理,观察组采用舒适化浅镇静护理,比较两组丙泊酚使用量、右美托咪定使用量、预后恢复情况、舒适度[舒适状况量表(GCQ)]评分及并发症发生率。结果:观察组丙泊酚和右美托咪定使用量均少于对照组,差异有统计学意义(P<0.05);观察组机械通气时间和ICU入住时间均短于对照组,差异有统计学意义(P<0.05);观察组GCQ评分高于对照组,差异有统计学意义(P<0.05);观察组谵妄、意外拔管发生率均低于对照组,差异有统计学意义(P<0.05)。结论:舒适化镇静护理应用于ICU重症脑外伤机械通气患者,可减少丙泊酚使和右美托咪定使用量,缩短机械通气和ICU入住时间,降低谵妄及意外拔管发生率,提高患者舒适度,其效果优于常规护理。
Objective:To observe application effects of comfortable shallow sedation nursing in ICU(intensive care unit)patients with severe traumatic brain injuries and mechanical ventilation.Methods:86 ICU patients with severe traumatic brain injuries and mechanical ventilation were selected as the research objects,and were divided into control group and observation group according to the random number table,43 cases in each group.The control group received routine nursing,while the observation group was given the comfortable shallow sedation nursing.The dosage of Propofol,the dosage of Dexmedetomidine,the prognosis and recovery,the score of comfort[general comfort questionnaire,(GCQ)]and the incidence of complications were compared between the two groups.Results:The dosages of Propofol and Dexmedetomidine in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The mechanical ventilation time and the ICU stay time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The GCQ score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Further,the incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:Comfortable sedation nursing for the ICU patients with severe traumatic brain injuries and mechanical ventilation can reduce the dosages of Propofol and Dexmedetomidine,shorten the duration of mechanical ventilation and the ICU stay time,reduce the incidence of delirium and other complications,and improve the patients’comfort levels.Moreover,it is superior to the routine nursing.
