摘要
目的探讨基于格林模式的健康教育在肾病综合征(NS)患儿主要照顾者中的应用效果。方法选取2019年12月至2020年12月苏州大学附属儿童医院肾脏免疫科收治的80对NS患儿及其主要照顾者为研究对象。按照随机数字表法分为对照组和观察组,每组40对。对照组患儿接受常规护理,其主要照顾者接受常规教育;观察组患儿接受常规护理的基础上,其主要照顾者接受基于格林模式的健康教育。比较2组主要照顾者的疾病知识掌握程度,以及2组NS患儿的服药依从性和血清三酰甘油(TG)、胆固醇(Cho)、白蛋白(ALB)的水平。结果干预1周后,2组主要照顾者的疾病相关知识评估问卷各项评分均显著高于同组干预前(P均<0.05),且观察组的上述评分均显著高于对照组(P均<0.05)。干预3个月后,2组患儿的Morisky用药依从性问卷(MMAS-8)评分均显著高于同组干预前(P均<0.05),且观察组的MMAS-8评分显著高于对照组(P<0.05)。2组患儿的TG、Cho水平均显著低于同组干预前(P均<0.05),ALB水平均显著高于同组干预前(P均<0.05);观察组的TG和Cho水平均显著低于对照组(P均<0.05),ALB水平显著高于对照组(P<0.05)。结论NS患儿的主要照顾者接受基于格林模式的健康教育可以有效提高其疾病知识掌握程度,提高NS患儿的服药依从性,改善NS患儿的TG、Cho和ALB水平。
Objective To explore the application effect of health education based on the PRECEDE-PROCEED model on the main caregivers of children with nephrotic syndrome(NS).Methods A total of80children with NS who were admitted to the Department of Nephrology and Immunology at Children.s Hospital of Soochow University from December2019to December2020and their main caregivers were enrolled.They were divided into a control group and an observation group by using a random number table,with40pairs in each group.The children in the control group received routine nursing,and their main caregivers received routine education.The children in the observation group received routine nursing,and their main caregivers received health education based on the PRECEDE-PROCEED model.The disease-related knowledge mastery of main caregivers of the two groups,as well as the medication compliance and levels of serum triacylglycerol(TG),cholesterol(Cho)and albumin(ALB)in NS children of the two groups were compared.Results After1week of intervention,the scores of various items of the disease-related knowledge questionnaire in main caregivers of the two groups were significantly higher than those before the intervention(all P<0.05),and the above scores of the observation group were significantly higher than those of the control group(all P<0.05).After3months of intervention,the Morisky medication adherence scale-8(MMAS-8)scores of children of the two groups were significantly higher than those before the intervention(both P<0.05),and the MMAS-8scores of the observation group were significantly higher than those of the control group(P<0.05).The levels of TG and Cho in the children of the two groups were significantly lower than those before the intervention(all P<0.05),and the level of ALB was significantly higher than that before the intervention(both P<0.05).The levels of TG and Cho of the observation group were significantly lower than those of the control group(both P<0.05),and the level of ALB of the observation group was significantly higher than that of the control group(P<0.05).Conclusion The health education based on the PRECEDE-PROCEED model for main caregivers of children with NS can effectively improve the mastery of disease-related knowledge,improve the medication compliance of children,and improve the levels of TG,Cho and ALB of children.
