摘要
目的:探讨参松养心胶囊与普罗帕酮对阵发性心房颤动消融术后患者近远期复发的影响。方法:选取2019年1月-2020年12月于江西省人民医院接受单次射频消融术治疗的216例阵发性房颤患者,按照随机数字表法将其分为西药组(给予普罗帕酮治疗)、中药组(给予参松养心胶囊治疗)和空白组(不给予任何药物),每组72例。比较三组心功能[左房内径(LAD)、左室舒张末内径(LVEDD)、左室收缩末内径(LVESD)、左室射血分数(LVEF)及舒张早期二尖瓣血流速度与舒张晚期二尖瓣血流速度比值(E/A)]、血清人N端前脑钠肽(NT-proBNP)、C反应蛋白(CRP)水平、复发率、复发时间、不良反应发生情况。结果:治疗后3个月,三组LAD、LVEDD、LVESD、LVEF、E/A比较,差异均有统计学意义(P<0.05)。治疗后3个月,三组血清NT-proBNP、CRP水平均低于治疗前,且中药组均低于西药组和空白组,西药组均低于空白组,差异均有统计学意义(P<0.05)。术后3、6个月,中药组复发率均低于西药组和空白组,复发时间均长于西药组和空白组,差异均有统计学意义(P<0.05)。中药组不良反应发生率均低于西药组和空白组,差异均有统计学意义(P<0.05)。结论:参松养心胶囊较普罗帕酮,更能改善阵发性心房颤动消融术后患者心功能,降低其术后近远期复发率,安全性更高,值得临床推广。
Objective:To investigate the effect of Shensong Yangxin Capsules and Propafenone on short-term and long-term recurrence of patients with paroxysmal atrial fibrillation after ablation.Method:A total of 216 patients with paroxysmal atrial fibrillation who received single radiofrequency ablation in Jiangxi Provincial People’s Hospital from January 2019 to December 2020 were selected,they were divided into western medicine group(Propafenone treatment),Chinese medicine group(Shensong Yangxin Capsules treatment)and blank group(no drug treatment)according to random number table method,with 72 patients in each group.The cardiac function[left atrial diameter(LAD),left ventricular end diastolic diameter(LVEDD),left ventricular end systolic diameter(LVESD),left ventricular ejection fraction(LVEF)and the ratio of early diastolic mitral valve flow velocity to late diastolic mitral flow velocity(E/A)],serum human N-terminal forebrain natriuretic peptide(NT-proBNP)and C reactive protein(CRP)level,recurrence rate,recurrence time and incidence of adverse reactions were compared among three groups.Result:3 months after treatment,there were significant differences in LAD,LVEDD,LVESD,LVEF and E/A among three groups(P<0.05).3 months after treatment,serum NT-proBNP and CRP levels in the three groups were lower than those before treatment,and those in the Chinese medicine group were lower than those in the western medicine group and blank group,those in the western medicine group were lower than those in the blank group,the differences were statistically significant(P<0.05).At 3 and 6 months after operation,the recurrence rates in the Chinese medicine group were lower than those in the western medicine group and the blank group,and the recurrence time in the Chinese medicine group were longer than those in the western medicine group and the blank group,the differences were statistically significant(P<0.05).The incidence of adverse reaction in the Chinese medicine group was lower than those in the western medicine group and the blank group,the differences were statistically significant(P<0.05).Conclusion:Compared with Propafenone,Shensong Yangxin Capsules can improve the heart function of patients with paroxysmal atrial fibrillation after ablation,reduce the recurrence rate in the near and long term after surgery,and have higher safety,which is worthy of clinical promotion.
