摘要
目的:探讨P波参数与心源性缺血性脑卒中的关系。方法:选取2020年1月-2021年4月东莞市滨海湾中心医院收治的247例缺血性脑卒中患者的病例资料进行回顾性分析。根据是否为心源性缺血性脑卒中将其分为对照组(n=146)和研究组(n=101),对照组为非心源性卒中,研究组为心源性卒中。比较两组各项P波参数、脑钠肽(brain natriuretic peptide,BNP)、D-二聚体水平。利用受试者工作曲线(ROC)评估PWD、BNP和D-二聚体对心源性缺血性脑卒中的预测价值;并利用Pearson相关性分析P波离散度(P wave dispersion,Pd)与BNP、D-二聚体的相关性。结果:研究组P波时限(P wave time limit,PWD)、P波最大时限(maximum time limit of P wave,Pmax)、Pd、BNP、D-二聚体水平均高于对照组(P<0.05)。PWD诊断心源性缺血性脑卒中的特异度(84.67%)、敏感度(83.49%)及曲线下面积(0.845)均优于BNP与D-二聚体的特异度(78.67%、64.37%)、敏感度(56.49%、79.67%)及曲线下面积(0.714、0.789)。经Pearson相关分析法显示,Pd与BNP水平、D-二聚体水平均呈正相关(P<0.05)。结论:Pd与BNP水平、D-二聚体水平均呈正相关,PWD对心源性缺血性脑卒中的诊断价值较高,临床应提高对P波参数异常患者的重视,积极控制病情的同时,降低缺血性脑卒中的发生。
Objective:To investigate the relationship between P wave parameters and cardiogenic ischemic stroke.Method:Data of 247 patients with ischemic stroke admitted to Binhai Bay Central Hospital of Dongguan City from January 2020 to April 2021 were retrospectively analyzed.Patients were divided according to whether they had cardiogenic ischemic stroke into control group(n=146)and study group(n=101),with non-cardiogenic stroke in the control group and cardiogenic stroke in the study group.P wave parameters,brain natriuretic peptide(BNP)and D-dimer levels were compared between the two groups.Receiver operating curve(ROC)was used to evaluate the predictive value of PWD,BNP and D-dimer for cardiogenic ischemic stroke.The correlation between P wave dispersion(Pd)and BNP and D-dimer was analyzed by Pearson correlation.Result:The levels of P wave time limit(PWD),maximum time limit of P wave(Pmax),Pd,BNP and D-dimer in the study group were higher than those in the control group(P<0.05).The specificity(84.67%),sensitivity(83.49%)and area under curve(0.845)of PWD in the diagnosis of cardiogenic ischemic stroke were better than those of BNP and D-dimer(78.67%,64.37%),sensitivity(56.49%,79.67%)and area under curve(0.714,0.789).Pearson correlation analysis showed that Pd was positively correlated with BNP and D-dimer levels(P<0.05).Conclusion:Pd is positively correlated with BNP level and D-dimer level.PWD has a high diagnostic value for cardiogenic ischemic stroke.Clinical attention should be paid to patients with abnormal P wave parameters,and the incidence of ischemic stroke should be reduced while the disease is actively controlled.
