摘要
目的采用口腔颌面锥形束CT研究下颌第一恒磨牙远中舌根的发生率及弯曲度,为临床治疗提供理论指导和依据。方法本试验为回顾性研究,具有双侧下颌第一恒磨牙的375例北京地区患者被纳入研究,采用口腔颌面锥形束CT的方法评估下颌第一恒磨牙远中舌根的发生率。测量104例患者180颗下颌第一恒磨牙的CBCT三维重建图像中远中舌根的颊舌向观弯曲角度以及邻面观最大弯曲角度。结果在375例下颌第一恒磨牙病例中,下颌第一恒磨牙远中舌根的发生率为27.7%,不同性别远中舌根的发生率无统计学差异(P=0.915),右侧发生率高于左侧,差异有统计学意义(P=0.031)。Schneider法测得在颊舌向观远中舌根的平均弯曲度为(15.74±9.42)°,而在邻面观平均弯曲度为(44.0±15.3)°,颊舌向观平面的弯曲度显著小于邻面观,差异有统计学意义(P<0.001)。结论了解并熟悉下颌第一恒磨牙远中舌根的发生率和弯曲度的特点,有助于提高根管治疗的成功率。
Objective To investigate the prevalence and root curvature of distoligual roots in the mandibular first molars in the Beijing area population.Methods The CBCT images of 375 patients with 750 mandibular first molars were collected and the prevalence of the distolingual root was determined with the axial,coronal and sagittal plane images reconstructed from the CBCT image data.The 180 three-rooted mandibular first molars from 104 patients were subsequently reconstructed into three-dimensional images,and the degrees of primary root canal curvatures of distolingual roots were measured by the Schneider method.Results The prevalence of distolingual roots in the mandibular first molars was 27.7%.There was no significant difference between male and female patients(P=0.915)while a significant difference existed between the right and left sides(P=0.031).The curvature of the distolingual root canal was greater in the proximal view(44.0±15.3)°than in the bucco-lingual view(15.74±9.42)°(P<0.001).Conclusions To understand and get familiar with the characteristics of the distolingual root of mandibular first molars is helpful to improve the success rate of root canal treatment.
