摘要
目的:从茯苓四逆汤加减治疗中晚期肝癌出发,探讨温阳化气法治疗中晚期肝癌的辨证诊疗思路并观察临床疗效。方法:选择中晚期原发性肝癌患者126例,采用回顾性分析的方法,按照纳排标准选择后以患者接受的治疗方式进行分组,分为介入组[(单纯肝动脉化疗栓塞(transarterial chemoembolization,TACE)]治疗)51例、中药组(单纯中药治疗)33例和联合组(中药联合TACE治疗)42例。评价3组临床疗效、肝功能指标、凝血酶原时间及T淋巴细胞亚群水平。结果:基于mRECIST标准的疗效评价,介入组和联合治疗组疗效明显优于中药组,且有更高的客观缓解率(P<0.05);肝功能指标、凝血酶原时间、T淋巴细胞亚群指标,治疗后联合组较介入组、中药组呈明显改善(P<0.05)。结论:基于温阳化气法使用茯苓四逆汤加减治疗中晚期肝癌患者可有效改善其肝功能和免疫状态,然而其抗肿瘤作用的疗效评价需要进一步研究。
Objective:To discuss the diagnosis and treatment based on syndrome differentiation of Yangwarming Qi-transforming method in the treatment of advanced hepatocellular carcinoma,and observe its clinical effects based on treating advanced hepatocellular carcinoma by modified Fuling Sini decoction.Methods:All 126 patients were selected and grouped by using retrospective analysis according to the inclusion and exclusion standard and therapeutic methods the patients accepted,they were divided into 51 cases in the intervention group(single TACE),33 cases of the herbal group(only herbs)and 42 cases in the combination group(herbs combined with TACE).To assess clinical effects,the indexes of liver function,prothrombin time and the levels of T lymphocyte subsets in three groups.Results:On the foundation of clinical assessment of mRECIST criteria,the intervention group and the combination group were superior to the herbal group in clinical effects notably,with higher objective remission rate(P<0.05);in the comparisons of the indexes of liver function,prothrombin time and the levels of T lymphocyte subsets,the improvements of the combination group were more notable than these of the intervention group and the herbal group after treating(P<0.05).Conclusion:The application of modified Fuling Sini decoction based on Yang-warming Qi-transforming method could effectively improve liver function and immunologic function in advanced hepatocellular carcinoma patients,however,clinical evaluation on its antitumor effects needs to be further studied.
