摘要
目的探讨螺旋CT(MSCT)增强扫描在胃癌诊断中的应用价值。方法选取114例胃癌患者作为研究对象,均行MSCT检查,并经病理组织检查明确病情。统计病理组织检查结果,以病理组织检查为"金标准",统计MSCT检查T、N、M分期结果,并分析T、N、M分期符合率及一致性。结果114例胃癌患者经病理组织检查证实12例T1期、20例T2期、49例T3期、33例T4期,26例N0期、33例N1期、38例N2期、17例N3期,90例M0期、24例M1期;114例胃癌患者经MSCT检查检出12例T1期、23例T2期、46例T3期、33例T4期,MSCT检查T分期符合率为85.09%(97/114),Kappa指数为0.786,与手术病理检查具有较好一致性(95%CI:0.672~0.900,P<0.05);114例胃癌患者经MSCT检查检出25例N0期、35例N1期、38例N2期、16例N3期,MSCT检查N分期符合率为89.47%(102/114),Kappa指数为0.856,与手术病理检查具有较好一致性(95%CI:0.746~0.965,P<0.05);114例胃癌患者经MSCT检查检出87例M0期、27例M1期,MSCT检查M分期符合率为93.86%(107/114),Kappa指数为0.823,与手术病理检查具有较好一致性(95%CI:0.640~1.006,P<0.05)。结论MSCT检查应用于胃癌患者中可准确判断TNM分期,可为临床制定针对性治疗提供依据。
Objective To explore the application value of enhanced spiral CT(MSCT)scanning in the diagnosis of gastric cancer.Methods A total of 114 patients with gastric cancer were selected as the research subjects,all of them underwent MSCT examination,and the disease was confirmed by pathological examination.The pathological tissue examination results were counted,and the pathological tissue examination was regarded as the"gold standard".The T,N,and M staging results of MSCT were counted,and the coincidence rate and consistency of T,N,and M staging were analyzed.Results 114 cases of gastric cancer were confirmed by histopathological examination in 12 cases of T1 stage,20 cases of T2 stage,49 cases of T3 stage,33 cases of T4 stage,26 cases of N0 stage,33 cases of N1 stage,38 cases of N2 stage,and 17 cases of N3 stage.90 cases of M0 stage,24 cases of M1 stage;114 cases of gastric cancer were detected by MSCT in 12 cases of T1 stage,23 cases of T2 stage,46 cases of T3 stage,33 cases of T4 stage,and the MSCT T staging coincidence rate was 85.09%(97/114),the Kappa index was 0.786,which was consistent with surgical pathological examination(95%CI:0.672~0.900,P<0.05);114 cases of gastric cancer were detected by MSCT in 25 cases of N0 stage and 35 cases of N1 stage,38 cases of N2 stage,16 cases of N3 stage,MSCT examination N staging coincidence rate was 89.47%(102/114),Kappa index was 0.856,which was in good agreement with surgical pathology(95%CI:0.746~0.965,P<0.05);87 cases of M0 and 27 cases of M1 were detected in 114 cases of gastric cancer by MSCT.The coincidence rate of MSCT for M staging was 93.86%(107/114),and the Kappa index was 0.823,which is better than surgical pathological examination.Consistency(95%CI:0.640~1.006,P<0.05).Conclusion MSCT can be used to accurately determine the TNM staging in patients with gastric cancer,and it can provide a basis for clinical development of targeted treatment.
