摘要
目的评估高龄胆结石患者应用腹腔镜手术治疗对于患者的治疗效果与安全性。方法选择医院在2020年8月—2021年8月接诊的96例高龄胆结石患者,根据随机数表法分为对照组与观察组,各48例。对照组给予开腹胆囊切除手术治疗,观察组给予腹腔镜手术治疗,将两组高龄胆结石患者的治疗效果开展比较。结果观察组手术时间(95.63±10.25)min、胃肠道功能恢复时间(12.13±4.12)h和住院时间(6.21±2.14)d短于对照组,观察组的术中出血量(50.13±9.52)mL少于对照组,差异有统计学意义(t=3.801、2.910、6.582、3.133,P<0.05)。观察组并发症发生率(6.25%)低于对照组,差异有统计学意义(χ^(2)=6.400,P=0.011)。观察组的IgA(3.88±0.42)g/L和IgG(15.55±5.77)g/L高于对照组,差异有统计学意义(t=10.900、3.825,P<0.001)。观察组GLQI生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论高龄胆结石患者应用腹腔镜手术治疗的效果显著,安全性较高。
Objective To evaluate the efficacy and safety of laparoscopic surgery for elderly patients with gallstones.Methods A total of 96 elderly patients with gallstones admitted by the hospital from August 2020 to August 2021 were selected,according to the random number table method,they were divided into control group and observation group,with 48 cases in each group,The control group was given open cholecystectomy,and the observation group was given laparoscopic surgery.The therapeutic effects of the two groups of elderly patients with gallstones were compared.Results The operation time of the observation group was(95.63±10.25)min,the recovery time of gastrointestinal function was(12.13±4.12)h,and the hospitalization time was(6.21±2.14)d shorter than the control group,the intraoperative blood loss of the observation group(50.13±9.52)mL was less than the control group,the difference was statistically significant(t=3.801,2.910,6.582,3.133,P<0.05).The incidence of complications in the observation group(6.25%)was lower than that in the control group,the difference was statistically significant(χ^(2)=6.400,P=0.011).The IgA(3.88±0.42)g/L and IgG(15.55±5.77)g/L of the observation group were higher than those of the control group,the difference was statistically significant(t=10.900,3.825,P<0.001).The GLQI quality of life score in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion Laparoscopic surgery for elderly patients with gallstones has significant effect and high safety.
