摘要
目的探讨重性抑郁障碍(MDD)患者失眠与认知功能的关系。方法选取2014年9月至2016年9月于首都医科大学附属北京安定医院门诊就诊的101例MDD患者为研究对象。采用中文版重复性成套神经心理状态测验(RBANS)评估患者的认知功能;采用汉密尔顿抑郁量表17项(HAMD-17)评估患者睡眠情况,根据睡眠障碍因子得分分成失眠组(n=45)和非失眠组(n=56)。比较两组患者的RBANS、HAMD-17得分,采用Kendall's tau-b相关及Pearson相关分析MDD患者认知功能和睡眠的相关性。采用分层回归分析MDD患者性别、婚姻状况、受教育年限、总病程、发作次数、失眠与词汇再识的关系。结果失眠组和非失眠组患者的HAMD-17得分[15.00(5.00,21.00)分比2.00(0,6.50)分]、RBANS总分[82.00(71.00,90.00)分比87.00(82.25,95.00)分]、词汇回忆得分[5.00(3.00,7.00)分比6.00(4.00,7.75)分]、词汇再识得分[19.00(18.00,20.00)分比20.00(19.00,20.00)分]比较,差异均有统计学意义(均P<0.05)。词汇回忆、词汇再识与失眠呈负相关(r=-0.170、-0.265;P<0.05);语义流畅性、词汇再识与睡眠障碍呈负相关(r=-0.198、-0.250;P<0.05)。分层回归分析结果显示,回归模型具有统计学意义(R=0.519,R2=0.269,F=5.712,P<0.05),失眠、婚姻状况、总病程、受教育年限是词汇再识的预测因素(P<0.05)。结论MDD患者的认知功能与失眠症状具有相关性。
Objective To explore the relationship between cognitive function and insomnia in patients with major depressive disorder.Methods A total of 101 patients with major depressive disorder(MDD),who were admitted to the outpatient clinic of Beijing Anding Hospital Affiliated to Capital Medical University from September 2014 to September 2016,were recruited.Repeatable Battery for the Assessment of Neuropsychological Status(RBANS)and the Hamilton Depression Scale-17(HAMD-17)were used to evaluate cognitive function and sleep quality.According to the sleep disturbance factor,they were divided into insomnia group(n=45)and noninsomnia(n=56)group.The RBANS and HAMD-17 scores of the two groups were compared.Kendall's tau-b correlation analysis and Pearson correlation analysis were used to explore the relationship between the cognitive function and insomnia in patients with MDD.Stratified regression analysis was used to analyze the relationship between sex,marital status,years of education,total course of disease,number of attacks,insomnia and list recognition in patients with MDD.Results There was a statistically significant difference in the HAMD-17 score[15.00(5.00,21.00)points vs 2.00(0,6.50)points],the total score of RBANS[82.00(71.00,90.00)points vs 87.00(82.25,95.00)points],list recall[5.00(3.00,7.00)points vs 6.00(4.00,7.75)points],list recognition[19.00(18.00,20.00)points vs 20.00(19.00,20.00)points]between insomnia group and non-insomnia group(all P<0.05).List recall score(r=-0.170,P<0.05)and list recognition score(r=-0.265,P<0.05)were negative correlated with insomnia;semantic fluency(r=-0.198,P<0.05)and list recognition score(r=-0.250,P<0.05)were negative correlated with sleep disturbance.The results of stratified regression analysis(R=0.519,R2=0.269,F=5.712,P<0.05)showed a statistically significant result that insomnia,marital status,total course of disease and years of education were the predictors of list recognition score(P<0.05).Conclusions Cognitive function is related to insomnia in patients with MDD.
