结核感染T细胞斑点试验、结核分枝杆菌抗体检查在诊断肺结核患者诊断中的应用比较 被引量:4

摘要 目的 探讨T细胞斑点试验(T-SPOT.TB)及结核分枝杆菌抗体(TB-Ab)在肺结核患者诊断中的应用效果。方法选取2019年2月至2021年2月我院收治的疑似肺结核患者200例,所有患者入院后均完成T-SPOT.TB及TBAb检查,并以“痰涂片及病原菌培养”作为“金标准”,并将T-SPOT.TB及TB-Ab检查结果与金标准进行比较,统计并计算不同检查方法的诊断效能(灵敏度、特异度、阳性预测值与阴性预测值)。结果 疑似200例肺结核患者经“痰涂片及病原菌培养”检查确诊57例,确诊率为28.50%。T-SPOT.TB检查确诊51例,诊断灵敏度为73.68%,特异度为93.71%;TB-Ab检查用于肺结核患者中最终确诊45例,灵敏度为59.65%,特异度为92.31%;T-SPOT.TB联合TB-Ab检查用于肺结核患者中确诊55例,断灵敏度为85.96%,诊断特异度为95.80%;T-SPOT.TB联合TB-Ab检查用于肺结核患者中诊断灵敏度、阳性预测值与阴性预测值均高于T-SPOT.TB、TB-Ab检查(P<0.05)。结论 T-SPOT.TB及TB-Ab检查用于肺结核患者中能获得较高的检出率,且两种方法联合测定能获得较高的诊断效能。 Objective To investigate the application effect of T-cell spot test(T-SPOT.TB) and mycobacterium tuberculosis antibody(TB-Ab) in patients with pulmonary tuberculosis. Methods 200 suspected tuberculosis patients admitted to our hospital from February 2019 to February 2021 were selected to receive T-SPOT.TB and TB-Ab test. The examination results were compared with "sputum smear and pathogen culture", which are regarded as "gold standard". Specifically, we evaluate the diagnostic efficacy of the two methods from the aspects of sensitivity, specificity, positive predictive value and negative predictive value. Results Among the 200 suspected tuberculosis patients, 57 cases were diagnosed by sputum smear and pathogen culture, and the diagnosis rate was 28.50%. Through T-SPOT.TB examination, 51 cases were confirmed, with the diagnostic sensitivity being 73.68% and specificity being 93.71%. Through TB-Ab test, 45 patients were confirmed, with the sensitivity being 59.65% and specificity of 92.31%. T-SPOT.TB combined with TB-Ab confirmed 55patients, with the sensitivity being 85.96% and specificity being 95.80%. In the diagnosis of pulmonary tuberculosis, specificity, positive predictive value and negative predictive value of T-SPOT. TB combined with TB-Ab were significantly higher than the separate detection(P<0.05). Conclusion T-SPOT.TB and TB-Ab achieve a high detection rate in pulmonary tuberculosis diagnosis, and the combination of the two obtains higher diagnostic efficiency.
