摘要
目的:探讨术前细针穿刺活检应用在甲状腺手术中的效果观察。方法:将2021年1月至2022年1月在本院外科行手术治疗的92例疑似甲状腺恶性肿瘤患者根据术前是否用细针穿刺活检进行分组,对照组术前未使用细针穿刺活检,观察组术前使用细针穿刺活检,以术后病理检查结果为金标准,对比两组的一般资料、局部复发及转移率、细针穿刺活检的敏感性、特异性、准确性、阳性预测值、阴性预测值。结果:观察组性别、年龄、病灶直径、病理类型相比无明显差异(P>0.05);观察组局部复发率、转移率均明显低于对照组(P<0.05);病理结果显示,阳性.32例,阴性14例;观察组穿刺活检显示,阳性30例(真阳性29例,假阳性1例),阴性16例(真阴性13例,假阴性3例);对照组结果显示,阳性29例(真阳性22例,假阳性7例),阴性17例(真阴性8例,假阴性9例);观察组诊断的敏感性、特异性、准确性、阳性预测值、阴性预测值均高于对照组(P<0.05)。结论:术前细针穿刺活检应用在甲状腺手术中的效果显著,其诊断的敏感性、特异性和准确性高,可指导临床选择适宜的手术切除方式及切除范围,提高手术根治效果,降低局部复发及转移率,具有积极的临床意义。
Objective:To investigate the effect of preoperative fine needle puncture biopsy in thyroid surgery.Methods:A total of 92 cases of patients with suspected thyroid malignancies who underwent surgical treatment in our hospital from January 2021 to January 2022 were grouped according to whether fine-needle needle biopsy was used before surgery.The control group did not use fine-needle needle biopsy before surgery,and the observation group used fine-needle needle biopsy before surgery,taking postoperative pathological examination results as the gold standard.The general data,local recurrence and metastasis rates,sensitivity,specificity,accuracy,positive predictive value and negative predictive value of fine needle biopsy were compared between the two groups.Results:There were no significant differences in gender,age,lesion diameter and pathological type in the observation group(P>0.05).The local recurrence rate and metastasis rate in the observation group were significantly lower than those in the control group(P<0.05).Pathological results showed that 32 cases were positive and 14 cases were negative.In the observation group,puncture biopsy showed 30 positive cases(29 true positive cases,1 positive case)and 16 negative cases(13 true negative cases,3 negative cases).The results of the control group showed that 29 positive cases(22 true positive cases,7 positive cases),17 negative cases(8 true negative cases,9 negative cases).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the observation group were all higher than that of the control group(P<0.05).Conclusion:Preoperative fine needle needle biopsy has a significant effect in thyroid surgery,and its diagnostic sensitivity,specificity and accuracy are high.It can guide the clinical selection of appropriate surgical resection method and resection range,improve the effect of radical surgery,reduce the rate of local recurrence and metastasis,and has positive clinical significance.
