摘要
目的探讨在体外受精-胚胎移植(IVF-ET)技术中单卵裂胚移植对多胎妊娠率的影响。方法714例接受助孕治疗的患者,根据移植胚胎数不同分为单卵裂胚移植(173例)及双卵裂胚移植(541例)。比较不同年龄段单、双卵裂胚移植患者的临床妊娠率、多胎妊娠率及早期流产率的差异。结果单卵裂胚移植多胎妊娠率4.65%低于双卵裂胚移植的37.16%,差异有统计学意义(P<0.05)。单卵裂胚移植<35岁、35~37岁、38~39岁患者的多胎妊娠率低于双卵裂胚移植患者,差异有统计学意义(P<0.05)。≥40岁患者双卵裂胚移植的临床妊娠率高于单卵裂胚移植,差异有统计学意义(P<0.05)。结论在IVF-ET中,<40岁患者如果有可供选择的优质胚胎,建议行单胚胎移植,≥40岁患者如果有足够可利用胚胎的情况下,尽量移植双胚胎。
Objective To discuss the effects of single cleavage-stage embryo transfer on Multiple pregnancy rate in in vitro fertilization and embryo transfer(IVF-ET)technology.Methods A total of 714 patients who received assisted pregnancy treatment were divided into single cleavage-stage embryo transfer(173 cases)and double cleavage-stage embryo transfer(541 cases)according to the number of transferred embryos.The differences in clinical pregnancy rate,Multiple pregnancy rate and early abortion rate of patients with single and double cleavage-stage embryo transfer in different age groups were compared.Results The multiple births rate 4.65%in single cleavage-stage embryo transfer was lower than 37.16%in double single cleavage-stage embryo transfer,and the difference was statistically significant(P<0.05).The Multiple pregnancy rate of patients aged<35,35-37 and 38-39 years old with single cleavage-stage embryo transfer was lower than that in patients with double cleavage-stage embryo transfer,and the differences were statistically significant(P<0.05).The clinical pregnancy rate of double cleavage-stage embryo transfer in patients≥40 years old was higher than that of single cleavagestage embryo transfer,and the difference was statistically significant(P<0.05).Conclusion In IVF-ET,single embryo transfer is recommended for patients<40 years old if there are available high-quality embryos,and double embryo transfer for patients≥40 years old if there are enough available embryos.
