摘要
目的探析米力农联合无创呼吸辅助通气治疗充血性心力衰竭的临床效果以及对患者凝血功能的影响。方法筛选该院2018年7月—2021年7月收治的80例充血性心力衰竭患者视为观察对象,按抽签法随机分为两组,对照组采用单一的米力农治疗,研究组给予米力农联合无创呼吸辅助通气治疗。对比两组的治疗效果,治疗前后的心功能指标[左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)]、心率(HR)、血浆N末端脑利钠肽原(NTpro-BNP)、同型半胱氨酸(Hcy)水平和凝血指标[血浆P-选择素(P-selectin)、D-二聚体(D-D)和血浆假性血友病因子(vWF)],并观察两组治疗后的血气指标[pH值、动脉二氧化碳分压(PaCO_(2))及动脉血氧分压(PaO_(2))的变化],分析不良反应情况。结果研究组的总有效率97.5%高于对照组75.0%,差异有统计学意义(χ^(2)=8.538,P=0.003);治疗后两组的心功能指标、HR、NTpro-BNP、Hcy、凝血指标以及血气指标差异有统计学意义(P<0.05);两组的不良反应比较差异无统计学意义(χ^(2)=1.013,P=0.314)。结论治疗充血性心力衰竭采用米力农联合无创呼吸辅助通气取得了较为满意的效果,提高了有效率,改善了心功能指标和凝血功能,降低HR、NTpro–BNP水平和不良反应,安全性高。
Objective To explore the clinical effect of milrinone combined with noninvasive respiratory assisted ventilation in the treatment of congestive heart failure and its effect on patients'coagulation function.Methods 80 patients with congestive heart failure admitted to the hospital from July 2018 to July 2021 were selected as the observation subjects and randomly divided into two groups by draw.The control group was treated with milrinone only,and the experimental group was treated with milrinone combined with noninvasive respiratory assisted ventilation.The treatment effects,the cardiac function indexes[left ventricular end systolic diameter(LVESD),left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD)],heart rate(HR),plasma N-terminal pro brain natriuretic peptide(NTpro BNP),homocysteine(Hcy)levels and coagulation indexes[plasma P-selectin,D-Dimer(D-D)and von Willebrand Factor(vWF)were compared before and after treatment.The blood gas indexes[changes of pH,arterial partial pressure of carbon dioxide(PaCO_(2))and arterial partial pressure of oxygen(PaO_(2))after treatment]were observed.The adverse reactions were analyzed.Results The total effective rate of the experimental group was 97.5%,which was higher than 75.0%of the control group,the difference was statistically significant(χ^(2)=8.538,P=0.003);there were significant differences in cardiac function indexes,HR,NTpro-BNP,Hcy,coagulation indexes and blood gas indexes between the two groups after treatment(P<0.05);there was no significant difference in adverse reactions between the two groups(χ^(2)=1.013,P=0.314).Conclusion Milrinone combined with noninvasive respiratory assisted ventilation in the treatment of congestive heart failure has achieved satisfactory results,improved the effective rate,improved the indexes of cardiac function and coagulation function,reduced the levels of HR,NTpro-BNP and adverse reactions.It is high security.
