摘要
目的探讨乳腺癌循环肿瘤DNA(circulating tumor DNA,ctDNA)基因突变与患者病理特征的相关性。方法选取2019年3月至2021年3月间安康市人民医院收治的60例乳腺癌患者(癌变组)和60例良性乳腺肿瘤患者(瘤变组),检测两组患者外周血ctDNA乳腺癌易感基因(BRCA)1/2突变情况,调查患者的病理资料并进行多因素分析。结果癌变组患者ctDNA BRCA1/2基因突变率分别为55.0%和70.0%,高于瘤变组的6.7%和6.7%,差异均有统计学意义(均P<0.05)。随着病理分期增加、分化程度减少和淋巴结转移,乳腺癌患者ctDNA BRCA1/2基因突变率逐步增加,差异均有统计学意义(均P<0.05)。以ctDNA BRCA1/2基因突变率为因变量,以患者临床病理学特征为自变量,多因素logistic回归分析显示,患者组织学分化程度、临床分期和淋巴结转移,均为ctDNA BRCA1/2基因突变率的影响因素(均P<0.05)。结论乳腺癌ctDNA BRCA1/2基因突变较常见,与患者临床病理特征相关,可用于辅助诊断与病情判定。
Objective To investigate the relationship between circulating tumor DNA(ctDNA)gene mutations and pathological features of breast cancer.Methods From March 2019 to March 2021,60 patients with breast cancer(canceration group)and 60 patients with benign breast tumors(neoplasia group)were selected at Ankang People’s Hospital.Breast cancer susceptibility gene BRCA1/2 were detected in peripheral blood.Pathological data were analyzed using multivariate analysis.Results The mutation rate of ctDNA BRCA1/2 gene was 55.0%and 70.0%,respectively in the canceration group,which was significantly higher than 6.7%and 6.7%,respectively of the neoplasia group(all P<0.05).The mutation rate of ctDNA BRCA1/2 gene increased significantly with increased pathological stages,decreased degree of differentiation and lymph node metastasis(all P<0.05).Multivariate logistic regression analysis showed that histological differentiation,clinical stages and lymph node metastasis were factors influencing the mutation rate of ctDNA BRCA1/2 gene(all P<0.05).Conclusion Breast cancer ctDNA BRCA1/2 gene mutation are common,and they are significantly associated with pathological features.It can be used for the diagnosis of breast cancer.
