摘要
目的探讨偏头痛与良性阵发性位置性眩晕(Benign Paroxysmal Positional Vertigo,BPPV)发病密切相关的前庭周围机制。方法我们采用反复臀部皮下注射硝酸甘油的慢性偏头痛大鼠模型,通过Western Blot的方法观察对照组、单纯慢性偏头痛模型组、羟苯磺酸钙干预组的内耳炎症因子表达,以及三组内耳耳石蛋白otoconin 90(OC90)的表达。并用扫描电镜观察耳石的形态。结果与对照组相比,偏头痛模型大鼠中与良性阵发性位置性眩晕发生明确相关的内耳耳石蛋白OC90表达明显减少,内耳炎症因子IL-6、TNF-α表达明显增多,进一步发现,羟苯磺酸钙可以抑制慢性偏头痛模型的炎症因子IL-6、TNF-α表达,且与偏头痛组大鼠相比,应用羟苯磺酸钙灌胃后的偏头痛大鼠内耳OC90表达明显增多。结论偏头痛会引起内耳炎症性改变以及耳石蛋白减少,羟苯磺酸钙可以干预这种改变,对偏头痛相关眩晕的发生机制具有重要的启示意义,并且提示羟苯磺酸钙对于偏头痛相关眩晕的治疗方面可能具有重要作用。
Objective To study the peripheral vestibular mechanism involved in migraine and benign paroxysmal positional vertigo(BPPV).Methods Chronic migraine was modeled by repeated subcutaneous injection of Nitroglycerin into the hip in rats.Expression of inflammatory factors as well as otoconin-90(OC90)in the inner ear were detected by Western blot in a control group,a migraine group and a migraine+Calcium Dobesilate group.Morphology of otolith was observed by scanning electron microscope.Results Compared with the control group,expression of OC90,which is closely related to BPPV,was significantly reduced in the inner ear,while expression of inflammatory factors IL-6 and TNF-αwas significantly increased inhibit,which was inhibited by calcium dobesilate.Intragastric administration of calcium dobesilate significantly increased expression of OC90 in the inner ear as compared with migraine rats without calcium dobesilate treatment.Conclusion Migraine can cause inflammatory response and decrease in the expression of OC90 in the inner ear,and calcium dobesilate can mitigate these changes,which has important implications in pathogenesis of migraine-related vertigo,such as vestibular migraine.The study suggests that calcium dobesilate may play an important role in the treatment of migraine-related vertigo.
