还原型烟酰胺腺嘌呤二核苷酸磷酸氧化酶2在局限性骨肉瘤组织中表达及与新辅助化疗效应及局部进展相关性研究 被引量:1

摘要 目的探讨局限性骨肉瘤组织还原型烟酰胺腺嘌呤二核苷酸磷酸氧化酶(NOX)2表达与新辅助化疗效应及局部进展(L-PD)的相关性。方法选取自2005年1月至2016年12月收治的134例局限性骨肉瘤患儿(骨肉瘤组)及同期53例多指畸形患儿的软骨组织样本(参考组)进行回顾性分析。分别采用免疫组织化学法及蛋白质印迹法检测组织NOX2蛋白表达。采用Huvos分级系统评估新辅助化疗反应,RECIST标准评估L-PD情况。结果骨肉瘤组组织NOX2蛋白表达率及相对表达量均高于参考组,差异均有统计学意义(P<0.05),且两种检测结果基本一致。经受试者工作特征曲线(ROC)分析,组织NOX2蛋白相对表达量用于诊断骨肉瘤的曲线下面积(AUC)为0.746。组织NOX2蛋白高表达组病灶数目更多,且新辅助化疗反应不良的患儿比例更高(P<0.05)。Kaplan-Meier生存曲线结果显示组织NOX2高表达者5年无L-PD时间短于低表达者(P<0.001)更高。同样地,单因素及多因素COX回归分析的结果显示组织NOX2蛋白高表达是骨肉瘤患儿5年L-PD预后不良的独立预测因子(P<0.001)。经ROC曲线分析,组织NOX2蛋白表达预测骨肉瘤患儿新辅助化疗反应不良及L-PD的AUC分别为0.774、0.774。结论组织NOX2蛋白表达对骨肉瘤患儿新辅助化疗不良及L-PD具有良好的预测效能,有望成为新型预测生物标志物。 Objective To analyze the correlation between the nicotinamide adenine dinucleotidevphosphate oxidase(NOX)2 expression and efficacy of neoadjuvant chemotherapy and local progressive disease(L-PD)in localized osteosarcoma.Methods 134 children with localized and primary osteosarcoma(osteosarcoma group)and 53 children with congenital polydactylia(reference group)admitted to our hospital from January 2005 to December 2016 were selected for retrospective analysis.Immunohistochemistry(IHC)and Western blot were used to detect NOX2 protein expression.The efficacy of neoadjuvant chemotherapy was assessed by Huvos grading system,and L-PD was assessed by RECIST criteria.Results The expression rate and relative expression of NOX2 protein in osteosarcoma group were higher than those in the reference group,the difference was statistically significant(P<0.05),and the two detection results were basically the same.According to receiver operating characteristic curve(ROC)analysis,the area under the curve(AUC)of tissue NOX2 protein relative expression for diagnosis of osteosarcoma was 0.746.The number of lesions was more in the group with high expression of NOX2 protein,and the proportion of children with adverse reaction to neoadjuvant chemotherapy was higher,the difference was statistically significant(P<0.05).Kaplan-meier survival curve results showed that patients with high tissue NOX2 expression had a higher poor survival prognosis(P<0.001)after 5 years without L-PD.Similarly,univariate and multivariate COX regression analysis showed that high tissue NOX2 protein expression was an independent predictor of poor prognosis of 5-year L-PD in children with osteosarcoma(P<0.001).According to ROC curve analysis,tissue NOX2 protein expression predicted the AUC of adverse response to neoadjuvant chemotherapy and L-PD in children with osteosarcoma were 0.774 and 0.774.Conclusion Tissue NOX2 protein expression is expected to be a new predictive biomarker for adverse response to neoadjuvant chemotherapy and L-PD in children with osteosarcoma.
