摘要
目的 探讨运用护理专案降低老年心血管重症患者非计划拔管发生率的效果。方法 由各层级护理人员及医师组成专案小组,确定“降低老年心血管重症患者非计划性拔管率”为活动主题,选取2019年3月至2020年5月CCU拔管的情况通过对结构、过程、结果的现况进行层别分析,找出解决对策并组织实施,整理、规范方法并实施。结果 通过本护理专案的实施达成100天内未发生非计划性拔管事件的目标,同时非计划性拔管发生率由改善前的8例(拔管率1.12‰)降低至改善后的1例(拔管率0.11‰),差异有显著性(χ^(2)=7.563,P=0.006)。通过本护理专案的实施,我科患者身体约束率由专案活动前的12.15%下降至活动后的10.60%,差异有显著性(χ^(2)=8.558,P=0.003)。通过本护理专案的实施,我科患者管道平均留置天数虽由专案活动前的5.88 d下降至专案活动后的5.57 d,但差异无显著性(χ^(2)=2.506,P=0.113)。结论 实施护理专案可有效降低非计划拔管事件的发生,进而提升护理安全及质量。
Objective To explore the effect of nursing program to reduce the incidence of unplanned extubation in elderly patients with severe cardiovascular diseases.Methods A special team composed of nursing staff and doctors at all levels determined "reducing the rate of unplanned extubation in elderly patients with serious cardiovascular diseases" as the theme of the activity.Through the hierarchical analysis of the structure,process and results of the extubation of CCU from March 2019 to March 2020,countermeasures were found and implemented,and methods were sorted out,standardized and implemented.Results The objective of no unplanned extubation occurred within 100 days was achieved through the implementation of the nursing project,and the incidence of unplanned extubation decreased from 8 cases before improvement(extubation rate 1.12‰)to 1 case after improvement(extubation rate 0.11‰),the difference was significant(χ~2=7.563,P=0.006).Through the implementation of this nursing project,the physical restraint rate of patients in our department decreased from 12.15% before the project activity to 10.60% after the activity,and the difference was significant(χ~2=8.558,P=0.003).Through the implementation of this nursing project,although the average indwelling days of patients in our department decreased from 5.88 days before the project to 5.57 days after the project,there was no significant difference(χ~2=2.506,P=0.113).Conclusion The implementation of nursing projects can effectively reduce the occurrence of unplanned extubation events,and improve the safety and quality of nursing.
