摘要
目的:探讨腹腔镜治疗胆囊十二指肠瘘的临床效果。方法:将2010年1月~2021年12月本院收治的70例胆囊十二指肠瘘患者按照数字表随机法分两组。开放模式组给予开腹手术,腔镜微创组实施腹腔镜手术。比较两组治疗前后机体应激因子、手术耗时以及总住院的时长、手术不良事件发生率。结果:治疗前两组机体应激因子比较,P>0.05,治疗后两组均显著升高,而腔镜微创组明显地低于开放模式组,P<0.05。腔镜微创组手术耗时以及总住院的时长均短于开放模式组,P<0.05。腔镜微创组手术不良事件有2例感染,发生率5.71%,开放模式组其有4例感染,3例胆漏和2例腹腔出血,发生率25.71%,P<0.05。结论:应用腹腔镜治疗胆囊十二指肠瘘是有效的。能有效地减轻患者的痛苦,降低术后并发症,缩短疗程和恢复期,且腹腔镜相对于开腹手术而言,可更好的减轻患者的机体应激水平。
Objective:To investigate the clinical effect of laparoscopic treatment of cholecystoduodenal fistula.Methods:70 patients with cholecystoduodenal fistula admitted to our hospital from January 2010 to December 2021 were randomly divided into two groups according to the number table method.The open mode group was given laparotomy,and the minimally invasive group was given laparoscopic surgery.The stress factors,operation time,total length of hospital stay,and incidence of adverse surgical events were compared between the two groups before and after treatment.Results:Before treatment,the stress factors of the two groups were compared,P>0.05,and after treatment,the two groups were significantly increased,while the endoscopic minimally invasive group was significantly lower than the open mode group,P<0.05.The operation time and total hospital stay in the minimally invasive endoscopic group were shorter than those in the open mode group,P<0.05.In the minimally invasive endoscopic surgery group,there were 2 cases of infection,the incidence rate was 5.71%,and there were 4 cases of infection,3 cases of bile leakage and 2 cases of abdominal hemorrhage in the open mode group,the incidence rate was 25.71%,P<0.05.Conclusion:Laparoscopic treatment of cholecystoduodenal fistula is effective.It can effectively alleviate the pain of patients,reduce postoperative complications,shorten the course of treatment and recovery period,and compared with open surgery,laparoscopy can better reduce the body stress level of patients.
