摘要
目的 探讨多层螺旋CT(MSCT)和低压空气灌肠在小儿异位阑尾炎诊断中的应用。方法 疑似异位阑尾炎患儿81例,术前均行MSCT和低压空气灌肠辅助定位及诊断,结果与手术及其病理诊断比较,分析MSCT和低压空气灌肠诊断异位阑尾炎与术后病理结果的一致性。结果 81例中,手术及其术后病理诊断为异位阑尾炎63例。MSCT诊断小儿异位阑尾炎的灵敏度为96.83%,特异度为94.44%,Kappa值为0.895。低压空气灌肠诊断小儿异位阑尾炎的灵敏度为95.24%,特异度为88.89%,Kappa值为0.825。结论 MSCT和低压空气灌肠在诊断小儿异位阑尾炎均有较高的灵敏度和特异度,与术后病理结果的一致性较高,可用于小儿异位阑尾炎的术前定位及诊断。
Objective To investigate the application of multi-slice CT(MSCT) and low pressure air enema in the diagnosis of ectopic appendicitis in children.Methods A total of 81 children with suspected ectopic appendicitis were examined with MSCT and low pressure air enema before surgery.The results were compared to those reported by surgery and postoperative pathological diagnosis.The diagnosis consistency of MSCT and low pressure air enema with postoperative pathological results was analyzed.Results Of 81 children, 63 children were diagnosed as ectopic appendicitis by the surgery and postoperative pathology.The sensitivity, specificity and Kappa value of MSCT in the diagnosis of ectopic appendicitis were 96.83%,94.44% and 0.895,respectively, which were highly consistent with those of postoperative pathological results.The sensitivity, specificity and Kappa value of low pressure air enema in the diagnosis of ectopic appendicitis were 95.24%,88.89% and 0.825,respectively, which were highly consistent with those of postoperative pathological results.Conclusion Both of MSCT and low pressure air enema examinations have higher sensitivity and specificity in the diagnosis of ectopic appendicitis in children, and can be used for the preoperative location and diagnosis of ectopic appendicitis in children.
