摘要
目的:探讨与分析经皮穿刺椎体成形术中骨水泥渗漏与骨水泥粘度的相关性。方法:选择2014年1月-2019年12月在本院诊治的椎体压缩骨折患者382例作为研究对象,根据随机数字表法把患者分为观察组与对照组各191例。所有患者通过经皮穿刺椎体成形术进行治疗,对照组与观察组分别给予低粘度骨水泥、高粘度骨水泥注射治疗,记录与随访骨水泥渗漏情况。结果:所有患者都顺利完成手术,2组骨水泥注入量对比差异不显著,无统计学意义(P>0.05)。观察组的手术时间、术后住院时间较对照组少(P<0.05)。与对照组相比,观察组术后1天、3天与7天的疼痛VAS评分均低(P<0.05)。观察组术后1个月与3个月的ODI评分与对照组相比,均显著升高(Z3<0.05)。观察组术后3个月发生骨水泥渗漏3例,发生率为1•6%;对照组中术后发生渗漏22例,发生率为11.5%,组间对比差异有统计学意义(P<0.05)。结论:高粘度骨水泥在经皮穿刺椎体成形术治疗椎体压缩骨折能减少骨水泥渗漏的发生,能减少手术创伤,促进缓解术后疼痛,有利于患者恢复椎体功能。
Objective:To explore and analyze the correlation between bone cement leakage and bone cement viscosity during percutaneous vertebroplasty.Methods:From January 2014 to December 2019,382 cases of patients with vertebral compression fractures diagnosed and treated in our hospital were selected as the research objects.All the cases were divided into the high viscosity group and low viscosity group with 191 cases each groups accorded to the random number table method.All patients were treated with percutaneous vertebroplasty,and the high-viscosity group and low-viscosity group with low-viscosity bone cement and high-viscosity bone cement injections,respectively,and the leakage of bone cement were recorded and followed up.Results:All the patients were propitiously completed the operation.There has no significant difference in the amount of bone cement compared between the two groups(P>0.05).The operation time and postoperative hospital stay of the high-viscosity group were less than those of the low-viscosity group(P<0.05).Compared with the low-viscosity group,the pain VAS scores of the high-viscosity group were all lower at 1,3 and 7 days after operation.The ODI scores of the high-viscosity group were higher than that of the low-viscosity group at 1 month and 3 months after surgery(P<0.05).In the high-viscosity group,there were 3 cases of bone cement leakage occurred at 3 months after the operation,the incidence were 1.6%;There were 22 cases occurred in the low-viscosity group,the incidence were 11.5%,and compared the difference between the groups were statistically significant(P<0.05).Conclusion:High-viscosity bone cement in the treatment of vertebral compression fractures with percutaneous vertebroplasty can reduce the occurrence of bone cement leakage,reduce surgical trauma,promote postoperative pain relief,and help patients restore vertebral body function.
