摘要
目的 观察超声引导下类固醇注射联合针刺治疗髂胫束综合征(ITBS)(风寒痹阻证)的疗效。方法患者60例按随机数字表法分为观察组与对照组各30例。两组均予针刺治疗,观察组加用超声引导下类固醇注射治疗。在治疗前和治疗7、14 d后对所有患者评估中医证候积分、数字疼痛评分(NRS)、股骨外侧髁髂胫束厚度、膝关节HSS评分、下肢功能评定(LEFS),日常生活能力评分(ADL)。结果 治疗后,观察组总有效率为97.67%,高于对照组的80.00%(P <0.05)。两组治疗前与治疗7、14 d后NRS评分、股骨外侧髁髂胫束厚度、膝关节HSS评分、LEFS、ADL比较,差异均具有统计学意义(P <0.01),且各时间点观察组和对照组比较,差异具有统计学意义(P <0.01)。结论 与单纯针刺相比,超声引导下类固醇注射联合针刺治疗ITBS(风寒痹阻证)的疗效更佳,能明显减轻疼痛,改善下肢运动功能,提高日常生活能力。
Objective:To study the effect of ultrasound-guided steroid injection combined with acupuncture on iliotibial tract syndrome of wind cold obstruction type.Method:Totally sixty patients with iliotibial band syndrome were randomly divided into a control group and an observation group,each with thirty cases.Both groups were treated with acupuncture,and the observation group was treated with steroid injection under ultrasound-guided.All patients were evaluated for TCM syndrome scale,numerical rating scale(NRS),iliotibial bundle thickness of lateral femoral condyle,HSS scale,leg-functional scale(LEFS)and activity of daily living scale(ADL)before treatment,at 7 and 14 days after treatment.Results:After treatment,the total effective rate of the observation group 97.67% was higher than that of the control group 80.00%(P < 0.05).The NRS,HSS,LEFS,ADL and iliotibial bundle thickness of the two groups before treatment and at 7,14 days after treatment were statistically significant(P < 0.01),and the differences between observation group and the control group were statistically significant at each time point(P < 0.01).Conclusion:Compared with acupuncture only,ultrasound-guided steroid injection combined with acupuncture is more effective in the treatment of wind-cold obstrcution type of ITBS,which can significantly reduce pain,improve lower limb motor function and improve the ability of daily living.
