摘要
目的总结归纳溃疡性结肠炎(ulcerative colitis,UC)的并发症,旨在加强UC并发症的防治意识,从而避免造成比原发病更严重的影响。方法回顾性分析2011年1月1日至2020年12月31日中国人民解放军联勤保障部队第960医院收治共459例UC住院患者的临床资料,分析其并发症的临床特点、发病情况及其防治情况。结果共纳入459例UC患者,男278例,女181例,男女比例为1.54∶1;年龄(42.92±14.96)岁(10~78岁)。2011年至2015年UC患者病例数316例;2016年至2020年病例数143例;459例UC患者中出现并发症的患者344例,男201例,女143例,男女比例为1.41∶1;年龄(43.47±15.02)岁。出现并发症患者第二季度最多,为110例,第一季度84例,第四季度80例,第三季度70例。344例合并并发症者病情严重程度分级:轻度125例,中度180例,重度39例;不同性别病变严重程度构成比较,差异无统计学意义(χ^(2)=3.54,P=0.171)。病变分布范围:直肠型38例,左半结肠型127例,全结肠型179例。伴有一种并发症者132例,一种以上并发症者212例;合并并发症的344例患者中:贫血190例(55.23%),低蛋白血症180例(52.33%),电解质紊乱139例(40.41%),肠道菌群88例(25.58%),肠道息肉疾病54例(15.70%),肿瘤疾病10例(2.91%),肛周疾病7例(2.03%),消化道出血6例(1.74%),消化道穿孔2例(0.58%),因穿孔出现急性弥漫性腹膜炎1例,回肠溃疡1例,腹腔感染1例,最终死亡患者2例,因UC导致1例孕妇出现提前早产,提前终止妊娠。结论UC会导致多种并发症,对并发症的及早干预、诊治能够通过对并发症的干预延缓UC的进展、恶化,从而有助于提高患者生活质量,部分患者在疾病早期不能对疾病的并发症作出准确的判断,因此未进行及时的处理,导致疾病预后较差,最终治疗效果欠佳,甚至导致死亡。
Objective To summarize and analysis on complications in ulcerative colitis(UC),understand it′s effects on all systems of body,then prevent and intervent them early.Methods The clinical data of 459 patients with UC from Jan.1st,2011 to Dec.31st,2020 were retrospectively analyzed.The clinical characteristics,incidence,prevention and treatment of complications were analyzed.Results A total of 459 patients of UC were included.The sexes of the patients were 278 males and 181 females.The ratio of males to females was 1.54∶1;the ages were mainly(42.92±14.96)years old.There were 316 patients from 2011 to 2015,and 143 patients from 2016 to 2020;344 patients combined with complications.The sexes of the patients were 201 males and 143 females.The ratio of males to females was 1.41∶1;and the ages were mainly(43.47±15.02)years old.There were 84 patients had complications in the first quarter;110 patients in the second quarter;70 patients in the third quarter;80 patients in the forth quarter.Depending on clinical symptoms and classification of severity of disease,125 cases,180 cases,39 cases patients from light-middle-severe were analyzed,respectively.There was no significant difference in the constituent ratio of disease severity between different sexes(χ^(2)=3.54,P=0.171).The data from distribution of lesions showed 38 cases in rectum,127 cases in left hemicolon,179 cases in whole colon.132 patients had one complication,212 patients more than one complications.Among the 344 complications in UC patients with 190 cases(55.23%)of anemia,and 189 cases were iron deficiency anemia,1 case was haemolytic anaemia;180 cases(52.33%)of hypoproteinemia,139 cases(40.41%)of electrolyte disorders;88 cases(25.58%)of intestinal dysbacteriosis,54 cases(15.70%)of intestinal polyposis,10 cases(2.91%)of cancer,7 cases(2.03%)of perianal disease,6 cases(1.74%)of gastrointestinal bleeding,2 cases(0.58%)of digestive tract perforation,1 case of acute diffuse peritonitis,1 case of ileal ulcers,1 case of abdominal infection,2 cases of death,1 case of preterm delivery.Conclusion UC can cause many complications,prevent and intervent complications early can delay the progression and intervention,and improve the quality of life.A lot of patients can not be correctly recognized and judged the complications of UC,so that they can not treat the disease in time.The results in poor prognosis,poor treatment,and even death.
