摘要
目的研究前列腺癌根治术(RP)联合淋巴结清扫术(ALND)治疗前列腺癌对患者术后尿控功能及复发的影响。方法回顾性分析南阳市第一人民医院2017年3月至2021年1月诊治的75例前列腺癌患者的临床资料,根据手术方式将患者分为对照组(31例)和研究组(44例)。对照组接受RP治疗,观察组接受RP联合ALND治疗。比较两组患者围术期指标、术后尿控功能、尿流动力学及并发症发射情况;术后随访1年,统计术后复发率及无复发生存时间。结果研究组手术时间[(148.74±28.94)min]长于对照组[(128.64±26.43)min],术中出血量[(291.34±45.67)ml]多于对照组[(270.19±42.86)ml],P<0.05;两组患者术后引流管拔除时间及住院时间比较差异未见统计学意义(P>0.05)。术后3个月,研究组尿控功能低于对照组(P<0.05);术后6、12个月,两组患者尿控功能比较差异未见统计学意义(P>0.05)。两组患者尿流动力学各项指标、术后并发症发生率及复发率比较差异均未见统计学意义(P均>0.05),研究组无复发生存时间[(10.54±0.76)个月]长于对照组[(10.05±1.25)个月],P<0.05。结论RP联合ALND虽可延长前列腺癌患者的无复发生存时间,改善术后肿瘤学结果,但会延长手术时间并增加术后并发症发生率,且对短期尿控功能具有一定的影响。
Objective To study the effects of radical prostatectomy(RP)combined with axillary lymph node dissection(ALND)on postoperative urinary continence function and recurrence in patients with prostate cancer.Methods The clinical data of 75 patients with prostate cancer treated in the First People’s Hospital of Nanyang from March 2017 to January 2021 were retrospectively analyzed,and they were divided into control group(31 cases)and study group(44 cases)according to the operation methods.Patients in control group were treated by RP,and patients in observation group were treated by RP combined with ALND.The perioperative indexes,postoperative urinary continence function,urodynamics and complications were compared between the two groups.The patients were followed up for 1 year,and the recurrence rate and recurrence-free survival time were counted.Results The operation time in the study group was(148.74±28.94)min,longer than the(128.64±26.43)min in the control group,and the intraoperative blood loss in the study group was(291.34±45.67)ml,higher than the(270.19±42.86)ml in the control group(P<0.05).There was no significant difference in postoperative removal time of drainage tube or hospitalization time between the two groups(P>0.05).Three months after surgery,urinary continence function in the study group was lower than that in the control group(P<0.05),but there was no significant difference in urinary continence function between the two groups 6 and 12 months after surgery(P>0.05).The differences in urodynamics indexes,incidence of postoperative complications and postoperative recurrence rate between the two groups were not statistically significant(all P>0.05).The recurrence-free survival time in the study group was(10.54±0.76)months,longer than the(10.05±1.25)months in the control group(P<0.05).Conclusions Although RP combined with ALND can prolong recurrence-free survival time,and improve postoperative oncological outcomes in prostate cancer patients,it will prolong operation time,and increase incidence of postoperative complications,with certain effects on short-term urinary continence function.
