摘要
目的探讨重组人脑利钠肽联合盐酸曲美他嗪片对扩张型心肌病伴心力衰竭患者冠脉血流储备及periostin蛋白、可溶性信号素4D(sSema4D)水平的影响。方法选取2018年10月至2020年10月我院收治的100例扩张型心肌病伴心力衰竭患者为研究对象,根据随机数字表法将其分为对照组与观察组,各50例。对照组采用盐酸曲美他嗪片治疗,观察组在对照组基础上联合重组人脑利钠肽治疗。比较两组的治疗效果、冠脉血流储备情况[静息状态冠脉血流速度(CFVb)、充血状态下最大冠脉血流速度(CFVh)、冠脉血流速度储备(CFVR)]、实验室指标(periostin蛋白、sSema4D)。结果观察组治疗总有效率高于对照组(P<0.05)。治疗后,两组患者的CFVb、CFVh及CFVR均高于治疗前,且观察组高于对照组(P<0.05)。治疗后,两组患者的periostin蛋白、sSema4D水平均低于治疗前,且观察组低于对照组(P<0.05)。结论重组人脑利钠肽联合盐酸曲美他嗪片治疗扩张型心肌病伴心力衰竭有利于提高治疗效果,改善患者心功能及冠脉血流储备,降低periostin蛋白、sSema4D水平。
Objective To investigate the influences of recombinant human brain natriuretic peptide combined with trimetazidine hydrochloride tablets on coronary blood flow reserve and levels of periostin protein, soluble semaphorin 4D(sSema4D) in patients with dilated cardiomyopathy complicated with heart failure. Methods One hundred patients with dilated cardiomyopathy complicated with heart failure admitted in our hospital from October 2018 to October 2020 were selected as the research objects. According to the random number table method, the patients were divided into control group and observation group, with 50 cases in each group. The control group was treated with trimetazidine hydrochloride tablets, and the observation group was treated with recombinant human brain natriuretic peptide on the basis of the control group. The therapeutic effect, coronary blood flow reserve situation [resting coronary flow velocity reserve(CFVb), maximum coronary flow velocity in hyperemia(CFVh), coronary flow velocity reserve(CFVR)] and laboratory indexes(periostin protein and sSema4D) were compared between the two groups. Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05). After treatment, the CFVb, CFVh and CFVR in the two groups were higher than those before treatment, and those in the observation group were higher than the control group(P <0.05). After treatment, the levels of periostin protein and sSema4D in the two groups were lower than those before treatment, and those in the observation group were lower than the control group(P<0.05). Conclusion Recombinant human brain natriuretic peptide combined with trimetazidine hydrochloride tablets in the treatment of dilated cardiomyopathy complicated with heart failure is beneficial to improve the therapeutic effect, improve the cardiac function and coronary blood flow reserve of patients, and reduce the levels of periostin protein and sSema4D.
