摘要
目的:探讨琥珀酰明胶注射液新的严重、新的一般不良反应的临床特点及原因,为药品安全性监测及临床用药提供参考。方法:分析德州市人民医院2019年以来的13例琥珀酰明胶注射液不良反应案例,并检索CNKI和Pubmed数据库,对2015年1月1日—2021年6月30日国内外文献88例琥珀酰明胶注射液不良反应病例进行整理。结果:通过对收集的101例琥珀酰明胶注射液所致不良反应分析发现,40~59岁年龄段发生不良反应数量最多,在原患疾病方面以骨科为主,不良反应发生类型以速发型为主[用药20 min内发生率为64.4%(65/101)],出现新的严重、新的一般不良反应发生率为18.1%(58/320)。结论:琥珀酰明胶注射液说明书中的不良反应描述内容较笼统,建议补充详细的不良反应表现,对明胶及肉类过敏的患者使用时应皮试,并有应急预案。药品监测部门、医疗机构应加强不良反应监测,促进安全合理用药。
Objective:To investigate the clinical characteristics and the reasons of adverse reaction by succinylated gelatin ingusion.Methods:Reported the 13 cases of succinylated gelatin adverse reaction since 2019,mining and finishing the datas of the adverse reaction by succinylated gelatin from CNKI and Pubmed on January 1,2015 to June 30,2021.Results:Through the analysis of 101 collected cases(including the number of cases in our hospital and cases reported in literature)caused by succinylgelatin injection.The adverse reactions caused by the largest number in 40 to 59 age group,the original is given priority to with the orthopaedic disease,the main type of adverse reaction is speed hairstyle[the 64.4%(65/101)of incidence rate within 20 minutes],emergenced new serious and new general adverse reaction incidence(18.1%,58/320).Conclusion:The description of adverse reactions in the pharmaceutical leaflet of Succinyl gelatin injection is more general,suggest added detailed adverse reaction performance,patients allergic to gelatin and meat should be skin test,ready to emergency rescue,in the process of application of drug monitoring departments,medical institutions should strengthen the adverse drug reactions monitoring,to promote safe and rational drug use.
