摘要
目的 探讨骨髓增生异常综合征患者的住院费用构成与住院费用变化趋势,为减轻个人和医保负担、有效控制医疗费用提供依据。方法 收集2014年1月1日至2019年12月31日某三甲专科医院骨髓增生异常综合征住院患者的有效病案资料11 801份,对住院费用构成及影响因素进行统计描述和非参数检验。结果 共纳入病案11 801份,患者平均年龄为43.87±17.14岁。性别、年龄、医疗付款方式、临床分型、是否初治、是否采用造血干细胞移植以及住院天数对骨髓增生异常综合征患者次均住院费用的影响具有统计学意义。2014年1月1日-2019年12月31日次均费用年定基比分别为103.36%、110.32%、124.07%、144.89%和151.08%。2019年次均费用最高,为人民币44 913.55元。住院费用构成中,药费、诊断费和血费占比位居前3位,其中药费占比最高,为53.31%,其次为诊断费和血费,分别为26.63%和6.40%。结论 骨髓增生异常综合征患者的总体次均住院费用、日均费用以及年均费用呈现上升趋势,治疗费和护理费上升趋势明显,药费占比最高,合理降低药费能有效控制医疗费用支出。
Objective To analyze the composition of hospitalization expenses in patients with myelodysplastic syndrome(MDS) and explore the changing trend of hospitalization cost.To provide reference for reducing the burden of individual and medical insurance and effectively controlling medical expenses.Methods Case data of patients with MDS in a three A and tertiary speciaty hospial from January 1,2014 to December 31,2019 were collected.Statistical description and non-parametric tests were performed on the composition of hospitalization expenses and their influencing factors.Results A total of 11 801 cases were included,and the average age of the patients was 43.87±17.14 years.Gender,age and medical payment method,clinical classification,whether or not they are newly treated,whether hematopoietic stem cell transplantation is used or not,and the length of hospital stay have statistically significant effects on the average hospitalization cost of patients with myelodysplastic syndrome.From January 1,2014 to December 31,2019,the annual fixed base ratios of the average expenses were 103.36%,110.32%,124.07%,144.89%,and 151.08%,respectively.The average cost per time in 2019 was the highest at RMB 44 913.55.In the composition of hospitalization expenses,medicine,diagnosis and blood accounted for the top three,of which medicine expenses accounted for the highest proportion at 53.31%,followed by diagnosis and blood expenses,which were 26.63% and 6.40%,respectively.Conclusions The overall average hospitalization expenses,daily average expenses,and average annual expenses of patients with myelodysplastic syndromes are showing an upward trend,and treatment expenses and nursing expenses have shown an obvious upward trend,of which medicine expenses account for the highest proportion.A reasonable reduction in medicine expenses can effectively control medical expenses.
