摘要
目的通过检测慢性阻塞性肺疾病(COPD)患者红细胞分布宽度(RDW)水平,预测其对COPD患者的严重程度和急性加重的临床价值。方法选取2019年10月至2020年10月在内蒙古科技大学包头医学院第二附属医院呼吸与危重症医学科就诊的COPD患者117例为研究对象,随访1年,观察其急性加重发生次数及肺功能变化情况,不同分级RDW的1年后肺功能变化情况,并进行cox回归分析及亚组分析。结果多因素回归分析显示,既往1年内急性加重与否和RDW水平是COPD患者再次发生急性加重事件的独立预后因素。RDW≥15%和<11%的COPD患者其急性发作的概率显著高于正常范围内患者,且肺功能1年内的恶化情况显著高于正常范围内的患者(P<0.05)。结论RDW水平是COPD患者急性加重事件发生的独立预后因素,同时其水平与肺功能的恶化程度显著相关。
Objective To predict the clinical value of severity and acute exacerbation of patients with chronic obstructive pulmonary disease(COPD)by detecting the level of red blood cell distribution width(RDW)level in patients with COPD.Methods A total of 117 patients with COPD admitted to the Department of Respiratory and Critical Care Medicine,the Second Affiliated Hospital of Baotou Medical College of the Inner Mongolia University of Science and Technology from October 2019 to October 2020 were included as the research subjects,and they were followed up for 1 year.The incidence of acute exacerbations and changes in pulmonary function were observed,the changes in pulmonary function after 1 year with different grades of RDW were observed,and cox regression analysis and subgroup analysis were conducted.Results Multivariate regression analysis showed that acute exacerbation and RDW level in the past 1 year were independent prognostic factors for the relapse of acute exacerbations in patients with COPD.The probability of acute exacerbation in patients with COPD with RDW≥15%and<11%was significantly higher than that in patients within the normal range,and the exacerbation of pulmonary function within 1 year was significantly higher than that in patients within the normal range(P<0.05).Conclusion RDW level is an independent prognostic factor of acute exacerbation events in patients with COPD,and its level is significantly associated with the degree of exacerbation of pulmonary function.
