摘要
目的了解2019—2020年西宁地区丙型肝炎病毒(hepatitis C virus,HCV)感染者HCV感染株基因型。方法采集2019—2020年青海省疾病预防控制中心门诊HCV感染者抗体阳性血清108份,采用实时荧光定量PCR法(RT-PCR)进行HCV-RNA阳性筛查,对HCV-RNA阳性标本进行RT-PCR特异性引物扩增后用直接测序法进行HCV基因片段的测序。结果108份HCV抗体阳性血清标本中共检测到77份阳性血清,经过RT-PCR扩增,共检出54份阳性标本,测序后得到25份核苷酸片段,再经序列测序进化分析得到2a型17份,占总数的68%;1b型8份,占总数的32%。2a型分布于两个不同的分支上,其中一个分支与ZX 2005-197株(KP 721897.1)在同一分支上,此柱为有明显输血史感染柱,另一分支与广州GZ 0160(206047)和KM 26(KF 586099)WL 14(KF 586224.1)的自愿献血者感染株为同一分支;8份1b型分布于3个不同的分支,其中一组与上海的标准株PR 50(HQ 912957.1)在同一分支上,另一组与美国的毒株KY 565218.1亲源关系较近,其余的序列单独形成一个分支。结论西宁地区HCV感染基因型以2a型为主,其次为1b型,此次检测结果与西宁地区其他关于HCV感染主要基因型研究报道有所不同。
Objective To understand the genotypes of hepatitis C virus(HCV)infective strains in patients infected with HCV in Xining region from 2019 to 2020.Methods Totally 108 antibody positive sera were collected from patients infected with HCV in the Outpatient Department,Qinghai Center for Disease Control and Prevention from 2019 to 2020,positive HCV-RNA was screened by real-time fluorescent quantitative PCR(RT-PCR),and HCV gene fragments were sequenced by direct sequencing after specific primer amplification of RT-PCR on positive HCV-RNA specimens.Results A total of 77 positive sera were detected from 108 HCV antigen-positive serum specimens,a total of 54 positive specimen were detected after RT-PCR amplification,25 nucleotide fragments were acquired after sequencing,and then 17 specimens of type 2 a were acquired via sequence sequencing and evolutionary analysis,accounting for 68%of the total,and there were 8 specimens of type 1 b,accounting for 32%,2 a genotypes were distributed in two different branches,one branch and Strain ZX 2005-197(KP 721897.1)were found to share the same branch and had significant histories of blood transfusion,and the other branch shared the same brach with Guangzhou GZ 0160(206047)and KM 26(KF 586099)WL 14(KF 586224.1)infectious strains from voluntary blood donors,8 types 1 b strains were distributed in 3 different branches,and one group was in the same branch with Shanghai standard Strain PR 50(HQ 912957.1).The other group was noted to have close affinity to the US Strain KY 565218.1,and the rest of the sequences form a separate branch.Conclusion The 2 a serves as dominant genotype of HCV infection in Xining region,followed by 1 b.The test results are different from other reports on major infected HCV genotypes in Xining region.
