摘要
射血分数保留型心力衰竭(heart failure with preserved ejection fraction,HFpEF)因其高发病率及高死亡率,已成为中国乃至全球最严重的公共卫生问题之一.由于HFpEF病理机制复杂并常伴随多种合并症,现有治疗手段临床获益较为有限.中医药在HFpEF治疗中具有潜在价值,可从多途径、多靶点、多层次表现出良好的疗效与优势.本文针对中医药在HFpEF防治中的用药规律进行了分析,并对其中高频率使用药物及药对进行分析,发现中医针对HFpEF的治疗,以益气活血、益气温阳、益气补阴和补气利水功效的中药及相关方剂为主.此外,针对HFpEF中医药研究面临的问题与挑战进行了初步探讨,提出应进一步规范HFpEF中医证候诊断标准,建立符合临床特点的基础研究动物模型,并积极运用现代多组学研究手段,不断提升中医药HFpEF研究质量,为中医药在HFpEF防治中的应用提供科学依据.
Heart failure with preserved ejection fraction(HFpEF)has become one of the most serious public health problems in China and even the world due to its high morbidity and mortality.Due to the complex pathological mechanism of HFpEF and its frequent comorbidities,the clinical benefits of existing treatments are limited.Traditional Chinese medicine has potential value in the treatment of HFpEF,which can show good efficacy and advantages from multiple approaches,multi-target and multi-level.This article analyzes the rules of traditional Chinese medicine in the prevention and treatment of HFpEF,as well as the high-frequency use of drugs and drug pairs.It was found that Chinese medicine for the treatment of HFpEF prefers to use the traditional Chinese medicine and relevant prescriptions that benefit Qi and activate blood circulation,promote Yang and supplement Yin,supplement Qi and induce diuresis.In addition,this paper discusses the problems and challenges faced by the research of HFpEF in traditional Chinese medicine,and propose that the traditional Chinese medicine syndrome diagnosis criteria for HFpEF should be further standardized,the basic research animal models should be established in line with the clinical characteristics,and the modern multiomics research methods should be actively used to continuously improve the quality of the research on HFpEF of traditional Chinese medicine.It may provide a scientific basis for the application of traditional Chinese medicine in the prevention and treatment of HFpEF.
