摘要
目的:应用数据挖掘技术探索针灸治疗血管神经性头痛的临床选穴规律及用穴特点。方法:检索中国期刊全文数据库(CNKI)、维普中文科技期刊数据库(VIP)和万方学术期刊全文数据库(Wanfang)自1986年4月2日—2020年10月1日针灸治疗血管神经性头痛的临床文献,严格按照纳入标准、排除标准筛选文献并经数据标准化处理。应用Microsoft Excel 2019软件建立针灸治疗血管神经性头痛的处方数据库,进行腧穴频次、归经、部位和特定穴分析,应用统计学软件SPSS Statistics 25.0进行系统聚类和因子分析。结果:共纳入63篇文献,192条处方,涉及腧穴79个,总频次589次,腧穴使用频次前3位依次为风池、百会和太冲;使用最频繁的经脉是胆经和督脉;腧穴主要分布在头部和下肢;特定穴以交会穴和五输穴为主。聚类分析得出风池-太阳-太冲-百会为重点穴位群集;因子分析共提取7个公因子。结论:针灸治疗血管神经性头痛以头部选穴为主并结合辨证取穴,总结出常用穴组规律,可为临床提供参考。
Objective: To explore the rules of acupoints selection and application characteristics of acupuncture and moxibustion in the treatment of angioneurotic headache by using data mining technology. Methods: Clinical literature of acupuncture and moxibustion in the treatment of angeurotic headache from April 2, 1986 to October 1, 2020 were retrieved from the databases of CNKI, VIP and Wanfang. The literature were screened strictly according to the inclusion and exclusion criteria, and were sorted out according to data standardization. Prescription database about acupuncture and moxibustion treatment for angioneurotic headache was established by Microsoft Excel 2019 software. Acupoints frequency, meridian tropism, points location and specific acupoints were analyzed by SPSS Statistics 25.0 for systematic clustering and factor analysis. Results: A total of 63 literature and 192 prescriptions were included, involving 79 acupoints, with a total frequency of 589 times. The top 3 acupoints were Fengchi(GB 20), Baihui(DU20) and Taichong(LR3). The most frequently used meridians were Gallbladder meridian and Du meridian. Acupoints were mainly distributed on head and lower extremities. Specific points were mainly confluent points and five-Shu points. Cluster analysis showed that GB20-Taiyang(EX-HN5)-Taichong(LR3)-Baihui(DU20) was the key acupoint cluster. Seven common factors were extracted by factor analysis. Conclusion: The points are mainly located on head in terms of acupuncture and moxibustion for angioneurotic headache, which are combined with points selection based on syndrome differentiation. This study also summed up the rules of common acupoints group, which can provide references and thoughts for clinical practice.
