摘要
目的探究无痛分娩配合预见性护理对初产妇产程及分娩结局的影响。方法选取98例初产妇,按照随机数字表法分为对照组与观察组,每组49例。两组均给予无痛分娩,观察组在此基础上配合预见性护理。比较两组分娩方式、产程、疼痛评分及不良妊娠结局。结果两组阴道助产率比较,差异无统计学意义(P>0.05),观察组自然分娩率高于对照组,剖宫产率低于对照组,差异有统计学意义(P<0.05)。观察组第一、二、三产程及总产程时间均明显短于对照组,疼痛评分低于对照组,差异有统计学意义(P<0.05)。观察组胎儿窘迫、新生儿窒息发生率分别为2.04%、4.08%,均低于对照组的16.33%、20.41%,差异有统计学意义(P<0.05),两组初产妇产后出血、发热、尿滞留及胎儿羊水吸入发生率比较,差异无统计学意义(P>0.05)。结论无痛分娩配合预见性护理可缓解初产妇疼痛感,提高顺产率,缩短产程,具体临床应用价值。
Objective To explore the effect of painless delivery combined with predictive nursing on the labor process and delivery outcome of primiparas.Methods 98 primiparas were randomly divided into control group and observation group,with 49 cases in each group.Both groups were given painless delivery and the observation group cooperated with predictive nursing on this basis.The delivery mode,labor process,pain score and adverse pregnancy outcome were compared between the two groups.Results There was no significant difference in the rate of vaginal delivery between the two groups(P>0.05).The rate of natural delivery in the observation group was higher than that in the control group,and the rate of cesarean section was lower than that in the control group,with statistical significance(P<0.05).The first,second and third stages of labor and total labor time in the observation group were significantly shorter than those in the control group,and the pain score was lower than that in the control group,with statistical significance(P<0.05).The incidence of fetal distress and neonatal asphyxia in the observation group was 2.04% and 4.08%,respectively,lower than 16.33% and 20.41 %in the control group,with statistically significant differences(P<0.05).There was no statistically significant difference in the incidence of postpartum hemorrhage,fever,urine retention and fetal amniotic fluid inhalation between the two groups(P>0.05).Conclusion Painless delivery combined with predictive nursing can relieve the pain of primipara,improve the rate of spontaneous labor,shorten the course of labor,and have specific clinical value.
