摘要
目的分析胰腺癌患者血清糖类抗原19-9(CA19-9)、异常凝血酶原(PIVKA-Ⅱ)、血管细胞黏附分子-1(VCAM-1)水平与临床分期的相关性。方法选取我院收治的122例胰腺癌患者作为胰腺癌组,回顾性分析患者临床资料,并纳人同期人院治疗的42例慢性胰腺炎患者作为胰腺炎组及36例健康体检者作为健康对照组。比较3组研究对象的血清CA19-9、PIVKA-Ⅱ、VCAM-1表达水平差异,并分析胰腺癌临床特征与血清CA19-9、PIVKA-Ⅱ、VCAM-1表达的关系。结果胰腺癌组患者血清CA19-9、PIVKA-Ⅱ、VCAM-1表达水平明显高于胰腺炎组及健康对照组(P<0.05),胰腺炎组患者血清CA19-9、PIVKA-Ⅱ、VCAM-1表达水平则明显高于健康对照组(P<0.05)。胰腺癌不同性别、年龄、吸烟史、肿瘤部位、分化程度、肿瘤大小、病理类型间血清CA19-9、PIVKA-Ⅱ、VCAM-1表达阳性率比较,差异无统计学意义(P>0.05);但不同TNM分期,不同淋巴结转移者间血清CA19-9、PIVKA-Ⅱ、VCAM-1表达阳性率存在显著差异(P<0.05)。胰腺癌Ⅰ~Ⅱ期患者血清CA19-9、PIVKA-Ⅱ、VCAM-1表达水平明显低于Ⅲ~Ⅳ期者(P<0.05),淋巴结转移者血清CA19-9、PIVKA-Ⅱ、VCAM-1表达水平明显高于非淋巴结转移者(P<0.05);Spearman秩相关分析也显示,胰腺癌TNM分期,淋巴結转移均与血清CA19-9、PIVKA-Ⅱ、VCAM-1表达水平呈显著正相关(P<0.05)。结论血清CA19-9、PIVKA-Ⅱ、VCAM-1在胰腺癌中均呈高表达,且表达水平与胰腺癌迁移、侵袭能力相关,有望成为胰腺癌诊疗的新指标。
Objective This study aimed to analyze the correlation between levels of serum carbohydrate antigen 19-9(CA19-9),abnormal prothrombin(PIVKA-Ⅱ)and vascular cell adhesion molecule-1(VCAM-1)and clinical stages in patients with pancreatic cancer.Methods The clinical data of 122 patients with pancreatic cancer in our hospital(pancreatic cancer group)were retrospectively analyzed,and 42 patients with chronic pancreatitis(pancreatitis group)and 36 healthy subjects with phtsical examination(healthy control group)admitted to the hospital during the same time period were included for comparison.The differences in expression levels of serum CA19-9,PIVKA-Ⅱ and VCAM-1 were compared among the three groups,and the relationship between clinical features of pancreatic cancer and expressions of serum CA19-9,PIVKA-Ⅱ and VCAM-1 was analyzed.Results The expression levels of serum CA19-9,PIVKA-Ⅱ and VCAM-1 in pancreatic cancer group were significantly higher than those in pancreatitis group and healthy control group(P<C0.05),and the expression levels in pancreatitis group were higher than those in healthy control group(P<0.05).There were no significant differences in the positive expression rates of serum CA19-9,PIVKA-Ⅱ and VCAM-1 among different gender,age,smoking history,tumor location,differentiation degree,tumor size and pathological type of pancreatic cancer(P〉0.05).However,there were significant differences in the positive expression rates of serum CA19-9,PIVKA-Ⅱ and VCAM-1 among patients with different TNM stages and different lymph node metastasis status(P<0.05).The expression levels of serum CA19-9,PIVKA-Ⅱ and VCAM-1 of patients with pancreatic cancer stage Ⅰ~Ⅱ were significantly lower than those of patients with stage Ⅲ~Ⅳ(P<0.05).The expression levels of serum CA19-9,PIVKA-Ⅱ and VCAM-1 in patients with lymph node metastasis were significantly higher than those in patients without lymph node metastasis(P<0.05).Spearman rank correlation analysis also showed that TNM staging and lymph node metastasis of pancreatic cancer were significantly positively correlated with expression levels of serum CA19-9,PIVKA-Ⅱ and VCAM-1(P<Z0.05).Conclusion All of CA19-9,PIVKA-Ⅱ and VCAM-1 may be related to the proliferation and migration of pancreatic cancer.Detection of their serum expression levels is beneficial for judging the occurrence and development of pancreatic cancer.
